Post-Surgery Care for External Hemorrhoids: What to Do About Swelling and Discharge - Surgery

Postoperative care for external hemorrhoids?

Three days ago, I underwent surgery for mixed hemorrhoids and a fistula.
There are two lumps at the anal opening, one large and one small.
Today, I rode my motorcycle to work; although it was painful, I managed to sit down.
When I got home, I noticed that the larger lump had a yellowish discharge and was leaking fluid.
What should I do?

Miss You, 30~39 year old female. Ask Date: 2021/09/09

Dr. Ke Fangxu reply Surgery

After hemorrhoid and fistula surgery, the wound cannot be completely healed within three days; there will be discharge for up to a month.
It is recommended to maintain local cleanliness and enhance sitz baths.
If there is redness, swelling, heat, or pain, please consult a physician for examination and treatment.

Post-surgery care for external hemorrhoids, especially after procedures like hemorrhoidectomy or fistulotomy, is crucial for recovery and minimizing complications. It’s not uncommon to experience swelling and discharge following such surgeries, but understanding how to manage these symptoms can significantly impact your healing process.

Understanding Your Symptoms
1. Swelling: After surgery, it is normal to have some swelling around the surgical site. This is part of the body’s natural healing process. However, excessive swelling can indicate complications such as infection or hematoma (a localized collection of blood outside of blood vessels).

2. Discharge: A yellowish discharge can be concerning. If it is clear or slightly yellow, it may be part of the healing process, especially if it is not accompanied by a foul odor. However, if the discharge is thick, green, or has a strong odor, it could be a sign of infection.

Immediate Steps to Take
1. Avoid Strain: It’s essential to avoid activities that put strain on the surgical area, such as sitting for prolonged periods, heavy lifting, or vigorous exercise. If you experience pain while sitting, consider using a cushion or a doughnut-shaped pillow to relieve pressure on the area.

2. Hygiene: Keep the area clean. After each bowel movement, gently clean the area with warm water. Avoid using harsh soaps or wipes that may irritate the skin. Pat the area dry with a clean towel or use a hairdryer on a cool setting.

3. Cold Compress: Applying a cold compress to the swollen area can help reduce swelling and alleviate pain. Make sure to wrap ice in a cloth to avoid direct contact with the skin.

4. Medications: Over-the-counter pain relievers such as acetaminophen or ibuprofen can help manage pain and reduce inflammation. Always follow the dosage instructions provided by your healthcare provider.

5. Dietary Adjustments: To prevent constipation, which can exacerbate pain and swelling, increase your fiber intake through fruits, vegetables, and whole grains. Staying hydrated is also crucial.

When to Seek Medical Attention
You should contact your healthcare provider if you notice any of the following:
- Increased Swelling: If the swelling worsens or does not improve after a few days.

- Foul Odor: If the discharge has a foul smell, which may indicate an infection.

- Severe Pain: If the pain becomes unbearable or does not respond to over-the-counter medications.

- Fever: If you develop a fever, which could be a sign of infection.

- Changes in Discharge: If the discharge changes from yellow to green or becomes thick and pus-like.

Follow-Up Care
It’s essential to attend all follow-up appointments with your surgeon or healthcare provider. They will assess your healing progress and address any concerns you may have. They may also provide additional treatments or interventions if complications arise.

Conclusion
Recovering from surgery for external hemorrhoids requires careful attention to post-operative care. Managing swelling and discharge effectively can lead to a smoother recovery. Always prioritize your comfort and hygiene, and don’t hesitate to reach out to your healthcare provider with any concerns. Remember, your health and well-being are paramount, and seeking help when needed is a vital part of the healing process.
