Wound healing issues

Hello doctor, I had a car accident on July 20th and sustained some abrasions.
Now, the wound on my knee does not hurt, but it is deep red and has an uneven texture.
It looks somewhat like it did right after the injury, but it feels like there is a transparent layer on the surface.
Is this a scar or is the wound still healing?

Niming, 10~19 year old female. Ask Date: 2021/09/11

Dr. Chen Jiaming reply Surgery

Based on your description, it seems more like scar tissue.
However, due to the greater degree of flexion in the knee area, scars that form there generally tend to hypertrophy and become thicker, appearing wrinkled when the knee is straightened.

When it comes to understanding wound healing, it's essential to recognize the various stages of healing and the characteristics of scars versus unhealed injuries. Based on your description, it sounds like you are observing a healing process that may include both scar formation and the potential for ongoing healing.

After an injury, the body goes through several phases of healing: hemostasis (stopping the bleeding), inflammation, proliferation (where new tissue forms), and remodeling (where the tissue matures and strengthens). The appearance of your knee injury, described as deep red with a bumpy texture and a transparent layer on top, suggests that you are likely in the later stages of the healing process.

Key Points to Consider:
1. Color and Texture: The deep red color indicates that there is still blood flow to the area, which is a good sign of healing. The bumpy texture can be due to the formation of granulation tissue, which is a normal part of the healing process. This tissue is often raised and can feel uneven as it matures.

2. Transparent Layer: The transparent layer you mentioned could be a sign of epithelialization, where new skin cells are forming over the wound. This is a positive indicator that your body is working to close the wound.

3. Scar Formation: Scars typically form as the wound heals and the body replaces damaged tissue with fibrous tissue. Scars can be raised, flat, or even indented, and their appearance can change over time. The fact that your injury feels uneven and has a different texture suggests that scar tissue may be forming, but it does not necessarily mean that the wound is not healing.

4. Pain and Sensation: The absence of pain is a good sign that the wound is healing. Pain often indicates inflammation or irritation, so if you are not experiencing pain, it suggests that the healing process is progressing well.

5. Monitoring for Infection: It's crucial to keep an eye on the wound for any signs of infection, such as increased redness, swelling, warmth, or discharge. If you notice any of these symptoms, it would be wise to consult a healthcare professional.

6. Timeframe for Healing: Wounds can take several weeks to months to heal fully, depending on their severity, location, and your overall health. Scars can continue to mature and change for up to a year after the initial injury.

Recommendations:
- Keep the Area Clean: Continue to clean the wound gently with saline or mild soap and water. Avoid using harsh chemicals that could irritate the area.

- Moisture Management: Keeping the wound moist can promote healing and minimize scarring. Consider using a non-stick dressing or a hydrogel to maintain moisture without causing maceration (over-saturation of the skin).

- Sun Protection: If the scar is exposed to sunlight, use sunscreen to protect it, as UV exposure can darken scars and make them more noticeable.

- Consult a Professional: If you are unsure whether the wound is healing properly or if you have concerns about the appearance of the scar, it’s always best to consult a healthcare provider. They can assess the wound and provide tailored advice or treatment options.

In conclusion, based on your description, it seems that your wound is healing, but the presence of a scar is likely. The healing process can vary significantly from person to person, so patience is key. If you have any concerns or if the wound changes in appearance or sensation, seek medical advice for further evaluation.
