Dizziness and discomfort after exercise?

Hello Doctor, I exercise for at least 30 minutes to 1 hour every day.
I usually go to bed before midnight and wake up at 7 AM.
My exercise routine typically starts around 4 PM and mainly consists of aerobic exercises combined with simple bodyweight training.
Recently, I have occasionally experienced dizziness not during exercise, but 5 to 10 minutes after exercising, particularly while resting or stretching.
Once the dizziness starts, I feel groggy for the entire evening, especially when bending over to wash my hair or face, which intensifies the sensation.
During these episodes of dizziness, I also experience a feeling of tightness in my chest and difficulty breathing, and lying down can make the dizziness worse.
I have been taking Eslizapine (易思坦) long-term.
I would like to know if the symptoms described could be caused by conditions other than autonomic nervous system dysfunction.
My psychiatrist has prescribed diphenidol tablets for me to take during sudden dizziness episodes.
Thank you.

Based on your description, there are four possible conditions.
First is orthostatic hypotension, which is characterized by a sudden drop in blood pressure when changing posture, leading to dizziness.
This typically resolves with rest, and there is no specific treatment other than making movements more slowly.

Second is anemia, which may require a blood test to check hemoglobin levels.

Third is fatigue, as physical activity from morning to evening can accumulate fatigue, making one prone to dizziness.
This can generally be managed by getting more rest.

Fourth is medication-related dizziness; you may want to consult with the prescribing physician to determine if your medications could be causing the dizziness.

As for diphenidol, it is commonly used to treat dizziness, and it may be worth trying to see if it helps.
Thank you for your inquiry.

Post-exercise dizziness can be a concerning symptom, especially when it is accompanied by feelings of chest tightness and difficulty breathing. Based on your description, it seems that you are experiencing dizziness shortly after your workouts, which can be attributed to several potential causes.

Possible Causes of Post-Exercise Dizziness:
1. Dehydration: One of the most common reasons for dizziness after exercise is dehydration. When you exercise, you lose fluids through sweat, and if you do not replenish these fluids adequately, it can lead to dizziness. Ensure you are drinking enough water before, during, and after your workouts.

2. Electrolyte Imbalance: Along with fluids, you also lose electrolytes (like sodium, potassium, and magnesium) during exercise. An imbalance can lead to symptoms such as dizziness, muscle cramps, and fatigue. Consider incorporating electrolyte-rich drinks or foods into your post-exercise routine.

3. Blood Pressure Changes: Exercise can cause fluctuations in blood pressure. If your blood pressure drops too low after exercise (a condition known as post-exercise hypotension), it can lead to dizziness. Since you mentioned that you are on medication (易思坦, which is likely a blood pressure medication), it’s essential to monitor your blood pressure regularly, especially after workouts.

4. Vasovagal Response: Some individuals experience a vasovagal response during or after exercise, which can cause dizziness, fainting, or lightheadedness. This is often triggered by standing up too quickly or straining during activities like washing your hair.

5. Autonomic Nervous System Dysfunction: Given your mention of autonomic nervous system dysregulation, it’s possible that your symptoms could be related to this condition. The autonomic nervous system controls involuntary functions, including heart rate and blood pressure regulation. Dysregulation can lead to symptoms like dizziness, especially during physical exertion.

6. Anxiety or Panic Attacks: The feelings of chest tightness and difficulty breathing you described could also be related to anxiety or panic attacks, which can occur during or after exercise, particularly if you are pushing yourself hard or feeling stressed.

Recommendations for Management:
1. Hydration: Make sure to drink plenty of fluids before, during, and after your workouts. Consider electrolyte drinks if you are exercising for extended periods or in hot conditions.

2. Gradual Cool Down: After your workout, take time to cool down gradually. Engage in light stretching or walking to help your body transition back to a resting state.

3. Monitor Blood Pressure: Regularly check your blood pressure, especially after exercise. If you notice significant drops, consult your healthcare provider about adjusting your medication.

4. Breathing Techniques: Practice deep breathing exercises to help manage feelings of anxiety or panic. This can also assist in alleviating chest tightness.

5. Consult a Specialist: Since you have ongoing symptoms and are on medication, it may be beneficial to consult with a cardiologist or a neurologist. They can conduct further evaluations to rule out any underlying conditions that may be contributing to your symptoms.

6. Medication Review: Discuss with your healthcare provider the possibility of reviewing your current medications, including diphenidol, to ensure they are appropriate for your symptoms and overall health.

7. Physical Therapy: If dizziness persists, consider working with a physical therapist who specializes in vestibular rehabilitation. They can provide exercises to help improve balance and reduce dizziness.

In conclusion, while post-exercise dizziness can be common, especially with factors like dehydration and blood pressure changes, it is essential to monitor your symptoms closely. Given your medical history and current medications, a thorough evaluation by a healthcare professional is advisable to ensure there are no underlying issues that need to be addressed.
