Questions regarding Biktarvy interactions?

Hello, doctor.
A few days ago, I had a potential exposure to risky behavior, and the next day I immediately consulted an infectious disease specialist and received HIV post-exposure prophylaxis (PEP) with the medication Biktarvy.
The infectious disease doctor advised me not to take supplements with metal ions, such as calcium tablets, and to avoid drinking milk.
I also asked if taking cold medicine would affect the treatment, and the doctor said it wouldn't.
However, after taking the medication for three days, I discovered that one of the cold medicines contained a stomach medication that has aluminum, magnesium, and calcium ions.
Although I usually eat a little food before taking the cold medicine, there were two days when the interval between taking Biktarvy and the stomach medication was less than two hours.
I would like to know if the interaction with the stomach medication could significantly impact the effectiveness of the HIV prevention treatment.
Thank you!

Hello, based on your description, theoretically, a one-hour interval should minimize any significant drug interactions and should not adversely affect the efficacy of the medication.
Thank you for your inquiry.

When it comes to the use of Biktarvy (a combination of bictegravir, emtricitabine, and tenofovir alafenamide) for post-exposure prophylaxis (PEP) against HIV, it is crucial to be aware of potential drug interactions that could affect its efficacy. Biktarvy is generally well-tolerated and effective, but certain medications and supplements can interfere with its absorption and effectiveness.

Your concern about the interaction between Biktarvy and the antacid containing aluminum, magnesium, and calcium is valid. Biktarvy, like many other antiretroviral medications, can have its absorption affected by the presence of multivalent cations (like those found in antacids). These cations can bind to the drug in the gastrointestinal tract, potentially reducing the amount of the drug that gets absorbed into the bloodstream. This is why healthcare providers often recommend spacing the administration of Biktarvy and any supplements or medications containing these minerals by at least two hours.

In your case, since you took the antacid within two hours of taking Biktarvy on two occasions, there is a possibility that the absorption of Biktarvy could have been compromised. However, whether this would lead to a significant reduction in the effectiveness of the PEP regimen depends on several factors, including the timing and dosage of both medications, as well as your overall health and adherence to the regimen.

It is important to note that while occasional interactions may not lead to complete treatment failure, they can increase the risk of suboptimal drug levels, which could potentially lead to HIV transmission if exposure occurs. Therefore, it is crucial to monitor for any signs of HIV infection and to follow up with your healthcare provider for further testing and guidance.

If you have concerns about the effectiveness of your PEP treatment due to these interactions, it would be advisable to discuss this with your healthcare provider. They may recommend additional testing to ensure that the PEP was effective or consider alternative strategies for HIV prevention if necessary.

In summary, while the interaction between Biktarvy and the antacid you took could potentially affect the drug's absorption, the overall impact on the effectiveness of your PEP regimen would depend on various factors. It is essential to maintain open communication with your healthcare provider regarding any medications you are taking and to follow their guidance on managing potential interactions. Always prioritize adherence to the prescribed regimen and seek medical advice if you have any concerns about your treatment.
