Time of conception
Hello, Doctor.
My menstrual cycle is irregular.
The last time I had my period was on June 25.
I had sexual intercourse with my husband on August 18 and August 22.
We did not use contraception on the 18th and did not fully use contraception on the 22nd, and there was no ejaculation inside.
I am currently diagnosed as being 6 weeks and 5 days pregnant.
Could you please tell me approximately when conception occurred? The due dates provided by two different hospitals are different, and I would like to ask for your advice.
Thank you, Doctor.
Mrs. Chen, 30~39 year old female. Ask Date: 2021/09/28
Dr. Cai Yonglong reply Obstetrics and Gynecology
Irregular menstrual cycles are assessed based on ultrasound data.
If September 28th corresponds to 6 weeks and 5 days of gestation, the estimated conception date would be around August 20th to August 26th, with some margin of error.

Understanding conception timing can be a complex process, especially for expecting mothers with irregular menstrual cycles. Based on the information you've provided, let's break down the details to help you understand when conception likely occurred and how to interpret the differing due dates given by the hospitals.
First, it’s important to note that the average menstrual cycle is about 28 days, but cycles can vary significantly from woman to woman. Irregular cycles can make it more challenging to pinpoint ovulation, which is the time when conception is most likely to occur. Ovulation typically happens about 14 days before the start of your next period. Given that your last menstrual period (LMP) was on June 25, if we assume a typical cycle length, ovulation would likely have occurred around July 9. However, since your cycles are irregular, this is just an estimate.
You mentioned having unprotected intercourse on August 18 and August 22. If we consider the ovulation date around July 9, the timing of your sexual activity suggests that conception could have occurred during this window. Sperm can live inside the female reproductive tract for up to five days, so if you ovulated shortly after your last intercourse on August 22, it is possible that conception occurred around that time.
Now, regarding the gestational age of 6 weeks and 5 days that you mentioned, this would place the estimated conception date around late July to early August, which aligns with the possibility of conception occurring after your sexual activities on August 18 or 22.
As for the differing due dates provided by the two hospitals, this can happen due to several factors. Different hospitals may use slightly different methods for calculating the due date. The most common method is to add 280 days (or 40 weeks) to the first day of your last menstrual period. However, if your cycles are irregular, some healthcare providers may adjust the due date based on ultrasound measurements taken during early pregnancy. Ultrasounds can provide a more accurate estimate of gestational age, especially if the measurements indicate that the fetus is larger or smaller than expected for the gestational age calculated from the LMP.
In conclusion, based on your last menstrual period and the timing of your sexual activity, it is likely that conception occurred in late July or early August. The differences in due dates between the two hospitals may be due to variations in calculation methods or adjustments based on ultrasound findings. It is always best to discuss these discrepancies with your healthcare provider, who can provide clarity based on your specific situation and any ultrasound results.
If you have further questions or concerns about your pregnancy, including any symptoms you may be experiencing or the health of your baby, do not hesitate to reach out to your healthcare provider. They can offer personalized advice and support throughout your pregnancy journey.
