Post-Laser Vision Issues: Double Vision and Blurriness Explained - Ophthalmology

After the laser treatment, I noticed double vision when looking at lights with one eye?

Hello, Doctor Gao.
I would like to ask you about something.
Last year, I underwent laser surgery for my myopia, and after some time post-surgery, I noticed that my right eye's vision became blurry (when measuring vision monocularly).
Although I can see around 0.8/0.9, it is not clear.
During the day, when using both eyes, I see some shadows.
When I use my phone, if the screen has a black background with white text, I can see the lower half of the text appearing as a ghost image.
The same happens with lights, including the moon (the lower part appears as a ghost image extending nearly halfway).
However, if there is a streetlight nearby, the moon appears quite clear.
I would like to know what could be causing these issues.

Dr. Gao Pilin reply Ophthalmology

Hello Ah Hao: The laser should not cause this issue; your condition is likely astigmatism.
I recommend that you visit an ophthalmologist for an examination.

After undergoing LASIK surgery, it is not uncommon for patients to experience various visual disturbances, including blurriness and double vision. These issues can be attributed to several factors related to the healing process of the cornea, the way the eyes work together, and the overall visual system.

Understanding the Symptoms
1. Blurriness in One Eye: The fact that your right eye measures around 0.8/0.9 but does not provide clear vision suggests that there may be residual refractive error or irregularities in the cornea. After LASIK, the cornea is reshaped to correct vision, but sometimes, the healing process can lead to uneven healing or scarring, which can result in blurriness.

2. Double Vision (Diplopia): The perception of seeing shadows or double images, especially when viewing bright lights or contrasting backgrounds, can be a sign of misalignment between the two eyes or issues with how the brain processes visual information from each eye. This can occur if one eye is focusing differently than the other, leading to a lack of fusion, where the brain struggles to combine the images from both eyes into a single coherent image.

3. Visual Distortions with Specific Backgrounds: The observation that you see a "ghost" image or shadow when looking at text on a black background or bright lights can be related to several factors:
- Contrast Sensitivity: After LASIK, some patients experience changes in their contrast sensitivity, which can make it difficult to distinguish between similar shades or brightness levels.

- Corneal Healing: If the cornea has not healed uniformly, it can create distortions in how light is refracted, leading to visual artifacts like halos or ghosting.

Potential Causes
- Residual Refractive Error: Sometimes, the laser treatment may not fully correct the refractive error, leading to ongoing issues with clarity.

- Dry Eyes: Post-surgery, many patients experience dry eyes, which can exacerbate visual disturbances. Dryness can lead to fluctuating vision and discomfort.

- Corneal Aberrations: Irregularities in the cornea's surface can lead to optical distortions, which might explain the ghosting effect you are experiencing.

- Binocular Vision Issues: If your eyes are not working together effectively, this can lead to diplopia or difficulty in focusing on objects, especially under varying lighting conditions.

Recommendations
1. Follow-Up with Your Eye Doctor: It is crucial to have a thorough follow-up examination with your ophthalmologist. They can assess your corneal health, check for any residual refractive error, and evaluate your binocular vision.

2. Consider Visual Therapy: If binocular vision issues are suspected, vision therapy may help improve how your eyes work together. This can be particularly beneficial if you are experiencing double vision.

3. Manage Dry Eyes: If dryness is contributing to your symptoms, using artificial tears or other lubricating eye drops can help alleviate discomfort and improve visual clarity.

4. Discuss Further Treatment Options: If residual refractive error is identified, your doctor may discuss options such as enhancement surgery or other corrective measures.

5. Be Patient: The healing process after LASIK can take several months. Many patients notice gradual improvements in their vision over time.

In conclusion, while experiencing blurriness and double vision after LASIK can be concerning, understanding the underlying causes can help in managing these symptoms effectively. Regular communication with your eye care provider is essential to ensure that any issues are addressed promptly and appropriately.
