Ear problems
Hello Doctor: I have some recent ear issues I would like to inquire about.
Typically, I experience some problems with my ears as autumn approaches, especially with my left ear, but this time it’s different.
The issue has shifted to my right ear, and sometimes, with certain movements or loud sounds, I hear a noise deep inside my ear (intermittently) that resembles a rolling or shifting sound.
Occasionally, I experience sharp pain when using ear cleaning tools, and I notice that when I touch the upper part of the ear canal, it not only hurts but also produces a sound.
However, this pain subsides, leaving behind the sensation of something moving deep inside, along with sounds similar to electromagnetic waves and faint crackling noises.
Additionally, my sleep quality has been poor, intermittent, and I have been directly exposed to a fan while sleeping.
I feel mentally exhausted, and sometimes I experience headaches, dizziness, and nausea.
I would like to ask the doctor if this is a common ear issue, or could it be middle ear inflammation or post-nasal drip? Or is it possible that there is something like a bug inside? I would appreciate your diagnosis.
Thank you!

More Info
Ear issues can be quite complex, and the symptoms you describe suggest a multifaceted problem that may involve the outer ear, middle ear, and possibly even the Eustachian tube. Let's break down your symptoms and potential causes, as well as when you should seek further medical help.
Symptoms Overview
1. Pain and Discomfort: You mentioned experiencing pain in your ear, particularly when touching certain areas. This could indicate inflammation or irritation in the ear canal or possibly an issue with the temporomandibular joint (TMJ), which is located near the ear.
2. Sounds in the Ear: The sensation of hearing sounds that resemble movement or rolling could be related to Eustachian tube dysfunction or even muscle spasms in the ear. This can happen when the Eustachian tube, which helps equalize pressure in the ear, is not functioning properly.
3. Sensitivity to Noise: Increased sensitivity to loud sounds can be a sign of hyperacusis, which is often associated with ear conditions or even stress and anxiety.
4. Tinnitus: The "electromagnetic" or "fine noise" sounds you describe could be tinnitus, which is often linked to hearing loss or ear damage.
5. Dizziness and Nausea: These symptoms can be indicative of vestibular issues, which are related to the inner ear's balance system.
Potential Causes
1. Eustachian Tube Dysfunction: This is a common issue, especially during seasonal changes. It can lead to pressure imbalances in the ear, causing pain and discomfort.
2. Middle Ear Infection (Otitis Media): If you have had a cold or sinus issues, fluid can build up in the middle ear, leading to infection and pain.
3. TMJ Disorders: Problems with the jaw joint can cause referred pain to the ear and may also lead to sounds when moving the jaw.
4. Tinnitus: This can be caused by a variety of factors, including exposure to loud noises, earwax buildup, or even stress.
5. Earwax Buildup: If you have been using tools to clean your ears, you may have pushed wax deeper into the ear canal, causing blockage and discomfort.
6. Nasal Congestion or Sinus Issues: Post-nasal drip or sinus infections can lead to ear problems, as the Eustachian tube can become blocked.
When to Seek Help
Given the complexity of your symptoms, it is advisable to seek medical attention if:
- Your symptoms persist for more than a week without improvement.
- You experience significant pain or discomfort that interferes with daily activities.
- You have a fever or notice any discharge from the ear.
- Your dizziness or nausea worsens or is accompanied by other concerning symptoms.
Recommended Actions
1. Visit an Ear, Nose, and Throat (ENT) Specialist: They can perform a thorough examination, including hearing tests and possibly imaging studies, to determine the underlying cause of your symptoms.
2. Avoid Self-Cleaning: Refrain from using tools to clean your ears, as this can exacerbate problems. Instead, consider using over-the-counter ear drops designed to soften earwax.
3. Manage Allergies or Sinus Issues: If you suspect that allergies or sinus problems are contributing to your ear issues, consider discussing this with your healthcare provider. They may recommend antihistamines or nasal sprays.
4. Stress Management: Since you mentioned poor sleep quality and fatigue, addressing stress and improving sleep hygiene may also help alleviate some of your symptoms.
In conclusion, while some ear issues can resolve on their own, persistent or severe symptoms warrant a visit to a healthcare professional for a comprehensive evaluation. Your health and comfort are paramount, especially as you navigate the challenges of potential ear-related issues.
