Frequent nocturia?
Hello doctor, for the past four months, I have been experiencing nocturia almost every night.
I usually wake up after about 3 to 4 hours of sleep to urinate (I notice morning erections when I wake up).
After urinating, I can go back to sleep until morning.
I do not drink a lot of water before bed and I also use the restroom once before sleeping.
During the day, I do not experience frequent urination, but I do feel a slight burning sensation when urinating.
I masturbate once a day.
I would like to ask the doctor if this could be a sign of benign prostatic hyperplasia (BPH)? Should I go to the urology department for an examination?

Hello: Symptoms related to the prostate typically begin to appear after the age of 40.
It is recommended to pay attention to lifestyle factors, ensure adequate sleep, engage in regular exercise, and maintain a daily water intake of over 2000cc, preferably from plain water.
Limit consumption of sugary beverages and avoid overly spicy foods.
If engaging in sexual activity, practice safe sex.
If symptoms do not improve within a week, it is advisable to consult a urologist.
Of course, if urinary pain becomes increasingly pronounced, seeking medical attention immediately is recommended.
Wishing you good health.

More Info
Nocturia, or the need to wake up during the night to urinate, can indeed be a sign of prostate enlargement, particularly in men over the age of 50. However, it is essential to consider other potential causes and symptoms before jumping to conclusions.
In your case, you mentioned experiencing nocturia almost every night for the past four months, waking up after about 3 to 4 hours of sleep to urinate. You also noted that you do not drink a lot of water before bed and that you urinate once before sleeping. The presence of morning erections (morning wood) indicates that your erectile function is likely intact, which is a positive sign. However, you also mentioned experiencing some pain during urination, which could indicate an underlying issue that may not be related to prostate enlargement.
Prostate enlargement, or benign prostatic hyperplasia (BPH), is a common condition that occurs as men age. It can lead to various urinary symptoms, including nocturia, increased frequency of urination, urgency, and difficulty starting or stopping urination. However, the presence of pain during urination is not typical for BPH and may suggest other conditions, such as a urinary tract infection (UTI) or prostatitis (inflammation of the prostate).
Given your symptoms, it would be prudent to consult a urologist for a thorough evaluation. A urologist can perform a physical examination, including a digital rectal exam (DRE) to assess the size of the prostate, and may recommend additional tests such as a urinalysis, urine culture, or imaging studies to rule out other potential causes of your symptoms.
In addition to BPH, other conditions that could lead to nocturia and urinary pain include:
1. Urinary Tract Infection (UTI): UTIs can cause symptoms such as burning or pain during urination, increased frequency, and urgency. They are more common in women but can occur in men as well.
2. Prostatitis: This condition can cause pelvic pain, painful urination, and sometimes flu-like symptoms. It can be acute or chronic and may require different treatment approaches.
3. Bladder Issues: Conditions such as overactive bladder or bladder stones can also lead to nocturia and urinary discomfort.
4. Diabetes: Uncontrolled diabetes can lead to increased urination, including nocturia, due to high blood sugar levels.
5. Medications: Certain medications, especially diuretics, can increase urine production and lead to nocturia.
6. Lifestyle Factors: Caffeine and alcohol consumption can irritate the bladder and contribute to nocturia.
In conclusion, while your symptoms could be indicative of prostate enlargement, the presence of pain during urination suggests that a more comprehensive evaluation is necessary. It is advisable to see a urologist who can provide a proper diagnosis and treatment plan tailored to your specific situation. Early intervention can help manage symptoms effectively and address any underlying conditions that may be contributing to your nocturia and urinary discomfort.
