Testicular issues
Hello Doctor, I would like to inquire whether the size and firmness of the testicles and epididymis can vary in different positions.
I usually feel that both testicles and the epididymis are larger and firmer when I am standing or sitting, while they feel smaller and softer after showering or lying in bed.
Is this normal? Additionally, I have noticed that the left testicle changes size more rapidly.
Generally, the left testicle feels slightly smaller than the right, and the left epididymis is harder to palpate compared to the right, which is easier to feel.
Is this normal? I previously consulted a urologist who performed a physical examination and said everything was fine.
Lastly, I can feel a lump behind and below my right testicle, which seems to extend along to the upper part of the testicle.
Is this the epididymis? That lump appears larger than the tubular structure leading to the upper testicle and changes size with the testicle.
It shouldn't be a tumor, right? Thank you, Doctor.

Hello: I would like to inquire if the size and firmness of the testicles and epididymis vary in different positions.
I usually feel that both testicles and epididymis are larger and firmer when I am standing or sitting, while they feel smaller and softer after taking a shower or lying in bed.
Is this normal? --》Normal.
Additionally, I would like to ask about the size changes; the left testicle seems to change size more rapidly.
I usually feel that the left testicle is slightly smaller than the right, and the left epididymis is harder to palpate, while the right epididymis is easier to feel.
Is this normal? I previously consulted a urologist who performed a physical examination and said there were no issues.
--》Normal.
Lastly, I would like to ask the doctor; I can feel a mass behind and below my right testicle, and it seems to extend upwards towards the testicle.
Is this the epididymis? --》It is possible that the mass is connected to the cord-like structure leading to the upper part of the testicle, and that mass is larger than the cord-like structure.
It changes size with the testicle, but it should not be a tumor, right? --》That would need a doctor's examination, but if it has not increased in size, it is likely not a concern.
Wishing you good health.

Understanding the changes in testicular size and hardness in different positions is an important aspect of male reproductive health. It is normal for the testicles and epididymis to exhibit variations in size and firmness based on body position and temperature.
When standing or sitting, the testicles may feel larger and firmer due to the contraction of the cremaster muscle, which elevates the testicles closer to the body for warmth and protection. This muscle response is a natural reflex to maintain optimal temperature for sperm production, which is typically a few degrees lower than body temperature. Conversely, when lying down or in a warmer environment, such as after a shower, the testicles may hang lower and feel softer and smaller. This is due to the relaxation of the cremaster muscle, allowing the testicles to descend further from the body, which helps to cool them down.
Regarding the observation that the left testicle seems to change size more rapidly than the right, it is not uncommon for one testicle to be slightly larger or smaller than the other. This asymmetry is generally normal and can vary from person to person. The left testicle is often positioned lower than the right, which can also contribute to the perception of size differences.
As for the difficulty in palpating the left epididymis compared to the right, this can be attributed to anatomical variations. The epididymis is a coiled tube located at the back of each testicle, and its size and position can vary. If a healthcare provider has examined you and found no issues, it is likely that what you are experiencing is within the range of normal anatomical variation.
The "lump" you feel behind the right testicle that extends to the upper part of the testicle is likely the epididymis itself. The epididymis can sometimes feel more prominent or larger, especially if there is a buildup of sperm or if it is slightly inflamed. It is important to note that the epididymis is a normal structure and should not be confused with a tumor. Tumors in the testicular area typically present as hard, irregular masses that do not change size with the position of the testicle.
If you notice any sudden changes in size, shape, or firmness of the testicles, or if you experience pain, discomfort, or any other concerning symptoms, it is crucial to seek medical attention. Regular self-examinations can help you become familiar with your anatomy and identify any changes that may require further evaluation.
In conclusion, the changes in size and hardness of the testicles and epididymis based on body position are normal physiological responses. Variations between the left and right testicles are common and usually not a cause for concern. However, maintaining awareness of your body and consulting with a healthcare professional if you have any doubts or concerns is always advisable.
