Could I be pregnant?
Hello Doctor: My girlfriend's last menstrual period started on September 7 and ended on September 11 (she has had regular cycles for the past three months).
We engaged in oral sex on September 23 (during her fertile window) and again on September 28.
During this time, my girlfriend performed oral sex on me, and afterward, my penis rubbed against her anal area.
I did not ejaculate, but there was some clear fluid from the bulbourethral glands.
I am concerned that her saliva, after licking my glans, may have come into contact with the base of my penis (since during the rubbing, the base of my penis was close to the area near the testicles and may have slightly brushed against the vaginal opening).
I would like to ask the doctor if the small amount of clear fluid from the bulbourethral glands, when mixed with saliva on the surface of my penis, could lead to pregnancy if it then came into contact with the vaginal opening.

The concern regarding the possibility of pregnancy from oral sex and pre-ejaculate fluid is a common one, and it’s important to clarify the facts surrounding this topic.
Firstly, pre-ejaculate fluid, also known as pre-cum, is a clear fluid that is released from the penis before ejaculation. This fluid is produced by the Cowper's glands and serves several purposes, including neutralizing acidity in the urethra and providing lubrication. One of the critical points to note is that pre-ejaculate fluid can contain sperm, although typically in much lower quantities than ejaculate. The presence of sperm in pre-ejaculate fluid can vary from person to person, and it is not guaranteed that it will contain viable sperm capable of fertilizing an egg.
In your scenario, you mentioned that your girlfriend performed oral sex and that there was some contact between your penis and her vaginal area after the oral sex. While it is theoretically possible for sperm to be transferred from the penis to the vaginal area through this kind of contact, the likelihood of this resulting in pregnancy is quite low, especially if there was no ejaculation involved.
Moreover, saliva does not contain sperm and is not conducive to sperm survival. When saliva mixes with pre-ejaculate fluid, it may dilute any sperm present, further reducing the chances of pregnancy. The vaginal environment is also acidic, which is not ideal for sperm survival.
Regarding the timing of your girlfriend's menstrual cycle, you mentioned that her last period started on September 7 and ended on September 11, with the potential ovulation window occurring around September 23 to September 28. If you had contact during this time, it is within the fertile window, but again, without ejaculation and with the factors mentioned above, the risk of pregnancy remains low.
It is also important to consider that while the risk of pregnancy in this situation is minimal, it is not zero. If you are concerned about the possibility of pregnancy, the best course of action is to wait for your girlfriend's next menstrual cycle. If her period is late, she should take a pregnancy test for confirmation.
In summary, while there is a theoretical risk of pregnancy from the scenario you described, the actual likelihood is quite low due to the absence of ejaculation and the dilution of sperm in saliva. If you and your girlfriend are looking to avoid pregnancy, it is advisable to use reliable contraception methods consistently. If you have further concerns or questions about sexual health and pregnancy risks, consulting a healthcare provider can provide personalized advice and information.
