Prepuce issues
Hello Doctor, I have an issue with phimosis, but I can retract the foreskin before and after an erection.
During an erection, half of the glans is exposed, and I can pull it back to reveal the entire glans, so I have no intention of undergoing circumcision.
A couple of days ago, I had intercourse with my girlfriend and noticed that my glans was very sensitive, making me ejaculate easily.
Therefore, yesterday I spent the entire day pushing the foreskin back behind the glans, thinking that this way, the glans would be in contact with my underwear and become less sensitive over time.
I also slept naked last night, keeping the foreskin pushed back all night.
However, this morning I noticed that the foreskin seems a bit swollen.
After researching online, it feels somewhat similar to paraphimosis, but it is not painful, and I can still retract the foreskin.
However, after pushing it back, it feels like the foreskin has become longer, and the skin in front of the glans has thickened.
So, I have returned to the normal state and am no longer pushing the foreskin back.
I would like to ask the doctor if this could lead to a permanent elongation of the foreskin, or will the swollen foreskin gradually return to its normal size? In the future, do you recommend continuing to push the foreskin back to stimulate the glans, or is it advisable not to do this for an entire day or longer?

Hello: Two days ago, I had intercourse with my girlfriend and noticed that my glans was very sensitive and I was easily aroused.
So yesterday, I spent the whole day retracting my foreskin behind the glans, thinking that this would allow the glans to come into contact with my underwear and become less sensitive over time.
Last night, I also slept without clothes, keeping the foreskin retracted all night.
However, this morning I noticed that the foreskin seems a bit swollen.
It is not recommended to keep the foreskin retracted for extended periods, as poor circulation can lead to swelling.
If you are unable to retract it back, you should go to the emergency room.
I found some information online that describes a sensation similar to incarceration, but there is no pain, and I can still retract the foreskin.
However, after retracting it, it feels like the foreskin has become longer, and the skin in front of the glans has thickened.
Therefore, I have returned to the normal state and have not retracted the foreskin again.
I would like to ask the doctor if this could lead to a permanent elongation of the foreskin.
--> No, the swollen foreskin will gradually return to its normal size.
Will the swelling decrease as circulation improves? --> Yes, as circulation improves, the edema will resolve.
Should I continue to retract the foreskin to stimulate the glans in the future? --> In your case, it is fine to do so during the day when you have sensation, but it is not recommended at night.
Wishing you good health.

More Info
It sounds like you are experiencing some concerns related to your foreskin and sensitivity, particularly after trying to desensitize your glans (the head of the penis) by keeping your foreskin retracted for an extended period. Let's break down your concerns and provide some guidance based on medical knowledge.
Understanding Your Situation
1. Foreskin Length and Sensitivity: You mentioned having a long foreskin that can be retracted both before and after an erection, which is a common situation. Many men have varying degrees of foreskin length, and it can affect sensitivity. The glans is typically more sensitive than other areas of the penis because it is covered by the foreskin in uncircumcised men, and exposing it can lead to increased sensitivity.
2. Desensitization Attempts: Your approach to desensitize the glans by keeping the foreskin retracted is not uncommon. Some men believe that prolonged exposure can reduce sensitivity over time. However, it’s essential to be cautious with this method, as excessive or prolonged retraction can lead to irritation or swelling, as you have experienced.
3. Swelling and Irritation: The swelling you noticed after keeping your foreskin retracted all night could be due to several factors, including friction, lack of lubrication, or prolonged exposure of the sensitive glans to clothing. The fact that you can still retract the foreskin and that it does not cause pain is a positive sign. However, if the swelling persists or worsens, it would be wise to consult a healthcare provider.
Addressing Your Concerns
1. Will the Swelling Cause Permanent Changes?: Generally, if the swelling is due to irritation or temporary inflammation, it should resolve on its own within a few days. The foreskin may appear slightly longer or thicker temporarily due to swelling, but this should not lead to permanent changes. If you notice that the swelling does not subside or if you experience pain, it would be advisable to seek medical advice.
2. Future Practices: While it is okay to experiment with desensitization techniques, moderation is key. Keeping the foreskin retracted for extended periods may not be advisable, especially if it leads to irritation or swelling. Instead, consider shorter intervals of exposure combined with adequate lubrication to minimize friction.
3. Consulting a Healthcare Provider: If you continue to have concerns about sensitivity, swelling, or any changes in your foreskin, it would be beneficial to consult a urologist. They can provide a thorough examination and offer personalized advice based on your anatomy and sexual health needs.
Additional Considerations
- Hygiene: Maintaining good hygiene is crucial, especially for uncircumcised men. Regularly cleaning under the foreskin can help prevent infections and irritation.
- Sensitivity Management: If sensitivity remains a concern, you might explore other methods, such as using desensitizing creams or sprays specifically designed for this purpose. However, always consult a healthcare provider before trying new products.
- Communication with Partners: Open communication with your partner about your experiences and any discomfort can also enhance your sexual experiences and reduce anxiety related to sensitivity.
In summary, while your approach to managing sensitivity is understandable, it’s essential to be mindful of how you do it to avoid irritation. If you have ongoing concerns, do not hesitate to reach out to a healthcare professional for tailored advice.
