Nosebleed
Hello, doctor.
I would like to ask the following: On the night of October 3rd, I drank alcohol and went to bed late (I usually go to bed a bit late).
Then, in the early morning, I suddenly had a nosebleed.
On the evening of October 5th, I also had a little nosebleed during dinner, but it was just a small amount.
Then, on October 6th and 7th, when I woke up and brushed my teeth, I noticed some small blood clots about the size of sesame seeds, which seemed to be coming from a cavity.
Could you please explain what is happening? Is it related to the sudden drinking and late sleeping? I visited a small clinic, and the doctor mentioned that it could be due to the weather and the thinning of the nasal membranes.
However, I think this might not be related to the bleeding from my teeth, so I wanted to consult you here.
Thank you, doctor.

Dr. Zheng Jueyi reply Otolaryngology
Hello: Common nosebleeds are often related to the rupture of microvessels in the nasal septal mucosa.
If you want to determine whether it is related to gum bleeding, it is recommended to visit a dentist for a thorough examination.
Wishing you safety and health.
Sincerely, Dr.
Jue-Yi Cheng, Director of the Department of Otolaryngology, Taoyuan General Hospital, Ministry of Health and Welfare.

Nosebleeds, or epistaxis, can be concerning, especially when they occur unexpectedly after drinking alcohol and staying up late. Your situation involves multiple factors that could contribute to the bleeding, including lifestyle habits, environmental conditions, and potential dental issues.
Firstly, alcohol consumption can lead to dehydration, which may dry out the mucous membranes in your nasal passages. When these membranes become dry, they are more susceptible to cracking and bleeding. Additionally, alcohol can cause blood vessels to dilate, which might increase the likelihood of bleeding, especially if you are already predisposed due to dryness or irritation.
Staying up late can also exacerbate the situation. Lack of sleep can weaken your immune system and overall health, making you more vulnerable to various issues, including nasal bleeding. Furthermore, if you are experiencing allergies or sinus issues, which can be aggravated by late nights and alcohol, this could also lead to increased nasal irritation and bleeding.
Regarding the small blood clots you noticed while brushing your teeth, it is essential to consider the possibility of dental issues, such as gum disease or cavities. If the bleeding is coming from a decayed tooth, it could indicate that the decay has progressed to the point where it is affecting the surrounding gum tissue. This could explain the blood clots you observed, especially if they are small and resemble sesame seeds. It is crucial to address any dental problems promptly, as untreated cavities can lead to more severe complications, including infections.
The doctor you consulted mentioned that the weather and the thinning of the nasal membranes could be contributing factors. Indeed, dry air, especially during certain seasons or in heated indoor environments, can lead to nasal dryness and increase the risk of nosebleeds. If you live in an area with low humidity or are exposed to dry air, using a humidifier at home can help maintain moisture in the air and prevent your nasal passages from drying out.
In summary, your nosebleeds could be attributed to a combination of factors: the dehydrating effects of alcohol, the potential for nasal membrane irritation from late nights and environmental conditions, and possible dental issues. Here are some recommendations to help manage and prevent future occurrences:
1. Stay Hydrated: Ensure you drink plenty of water, especially if you consume alcohol. This will help keep your mucous membranes moist.
2. Limit Alcohol Intake: Consider reducing your alcohol consumption, particularly before bedtime, to minimize its dehydrating effects.
3. Improve Sleep Hygiene: Establish a regular sleep schedule and create a relaxing bedtime routine to improve your overall sleep quality.
4. Use a Humidifier: If you live in a dry environment, using a humidifier can help maintain moisture in the air and prevent nasal dryness.
5. Dental Check-Up: Schedule an appointment with a dentist to evaluate any potential dental issues that may be causing bleeding from your gums or teeth.
6. Monitor Allergies: If you have a history of allergies, consider consulting an allergist to manage symptoms that could contribute to nasal irritation.
If your nosebleeds persist or worsen, or if you experience other concerning symptoms, it is essential to seek further medical evaluation. A healthcare professional can conduct a thorough examination and provide tailored advice based on your specific situation.
