Menstrual issues

Hello Dr.
Wu: My menstrual cycle has usually been around 30 days, but from August to September, it was only 26 days, which was early.
However, from September to October, it extended to a 35-day cycle.
This month, on the first and second days of my period, there was only light spotting, and it wasn't until the third day that I experienced a heavier flow.
On the fourth day, the flow decreased to just a thin line of blood on the pad, and by the fifth day, there was no blood on the pad, only a small amount when I wiped.
Could you please advise what might be causing this issue?

SC, 30~39 year old female. Ask Date: 2021/10/11

"Menstrual irregularities" have numerous causes, including lifestyle habits, stress, age, vaginal infections, benign gynecological conditions, and malignant tumors, among others.
Therefore, it is recommended that you visit a professional obstetrician-gynecologist for examination and evaluation.

Irregular menstrual cycles can be concerning and may arise from a variety of factors. It's important to understand that menstrual cycles can fluctuate due to numerous reasons, and these changes can be influenced by both physiological and environmental factors.
Firstly, a typical menstrual cycle ranges from 21 to 35 days, with an average of about 28 days. However, variations in cycle length, flow intensity, and duration can occur due to several reasons. In your case, the cycle changes from 30 days to 26 days, then extending to 35 days, along with variations in flow intensity, suggest that your body may be experiencing some hormonal fluctuations.

Hormonal imbalances are one of the most common causes of irregular menstrual cycles. The menstrual cycle is regulated by a complex interplay of hormones, including estrogen and progesterone. Factors that can disrupt this balance include stress, significant weight changes (either gain or loss), excessive exercise, and underlying health conditions such as polycystic ovary syndrome (PCOS) or thyroid disorders. Stress, in particular, can have a profound impact on your menstrual cycle, as it affects the hypothalamus, which regulates the hormones responsible for menstruation.

Another consideration is the possibility of perimenopause, especially if you are in your late 30s or 40s. This transitional phase leading up to menopause can cause irregularities in your menstrual cycle, including changes in flow and cycle length.
In your description, the changes in flow—starting with light spotting, then heavier flow, and finally tapering off—could indicate anovulation (when the ovaries do not release an egg) or hormonal fluctuations. Light bleeding or spotting at the beginning or end of a period can sometimes be attributed to hormonal changes or even implantation bleeding if pregnancy is a possibility.

It’s also worth considering lifestyle factors. If you've experienced any significant changes in your routine, such as increased stress, changes in diet, or alterations in physical activity levels, these could also contribute to irregular cycles.
Given the complexity of menstrual health, it is advisable to consult with a healthcare provider if these irregularities persist. They may recommend a thorough evaluation, including blood tests to check hormone levels, a pelvic exam, or imaging studies if necessary. Keeping a menstrual diary can also be beneficial, as it helps track your cycles, symptoms, and any other relevant factors that may assist your healthcare provider in diagnosing the issue.

In summary, while occasional irregularities in menstrual cycles can be normal, persistent changes warrant further investigation. Hormonal imbalances, lifestyle factors, and potential underlying health conditions are all important considerations. Seeking medical advice can help clarify the situation and ensure that any necessary interventions are made to support your reproductive health.
