Sexual dysfunction
Hello, doctor.
I have a question.
During masturbation, I experience numbness in my penis and have lost the usual sensitivity and pleasure, or the pleasure feels weak, making it take a long time to reach ejaculation.
I often feel like I can't ejaculate at all.
When I visited a doctor, they prescribed me colistin antibiotics, suspecting a bacterial prostatitis.
However, two previous doctors had already prescribed antibiotics, specifically levofloxacin and doxycycline, which I took for a week each.
Later, the doctors said there was no need for a follow-up visit and just advised me to pay attention to my lifestyle, but my problem persists.
If I have a bacterial infection in the prostate, did the previous two antibiotics not work? I read online that the only antibiotics effective for treating bacterial prostatitis are trimethoprim and quinolone.
I'm really conflicted about whether to continue taking antibiotics with this doctor.
The doctors have only done urine tests and blood tests, but strangely, the urine test results from hospital A and hospital B are different; one says there is an infection and inflammation, while the other says there isn't.
I'm starting to feel frustrated with the doctors, as it seems they can't even make a proper diagnosis and are just trying different medications.

Hello: The information provided here is for reference only and cannot replace a physician's examination.
1.
During masturbation, if you experience numbness in the penis and a loss of the usual sensitivity and pleasure, or if the feeling of euphoria is diminished to the point where it takes a long time to achieve ejaculation, and you often feel as if you cannot ejaculate—are there any other symptoms? Prostatitis commonly has other symptoms, and the treatment duration is long.
For more information on prostatitis, you can refer to: http://www.shutien.org.tw/library_detail.aspx?healthid=3962.
2.
Ciprofloxacin belongs to the Quinolone class; a general urinalysis can be used as a reference, and abnormalities may normalize after medication.
3.
It is advisable to consistently visit a medical center, preferably the same hospital, as this increases the likelihood of comparing examination and test data effectively.
4.
Pay attention to your lifestyle, including diet, rest, and exercise.
Wishing you good health.

Erectile dysfunction (ED) is a complex condition that can stem from various causes, including psychological, neurological, hormonal, and vascular factors. Your experience of numbness during masturbation and diminished sensitivity can be distressing and may indicate underlying issues that need to be addressed.
Firstly, it's important to understand that the sensation of numbness and reduced pleasure during sexual activities can be linked to several factors. These may include anxiety, depression, or even physical conditions affecting blood flow or nerve function. In your case, the fact that you have been prescribed antibiotics for suspected prostatitis (inflammation of the prostate) suggests that your healthcare providers are considering a possible infection or inflammation as a contributing factor to your symptoms.
Prostatitis can be caused by bacterial infections, and the treatment typically involves antibiotics. However, it is crucial to note that not all cases of prostatitis are due to bacterial infections. Chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS) is a common condition that may not respond to antibiotics, as it can be non-bacterial in nature. This could explain why you have not experienced relief after taking doxycycline and levofloxacin, which are commonly prescribed antibiotics for bacterial prostatitis.
Regarding your concerns about the effectiveness of the antibiotics you have taken, it is true that certain antibiotics are more effective against specific types of bacteria. Trimethoprim and fluoroquinolones (like ciprofloxacin and levofloxacin) are often recommended for bacterial prostatitis. However, if your symptoms persist despite treatment, it may indicate that the underlying cause is not bacterial, or that the infection is resistant to the antibiotics prescribed.
The discrepancies in urine test results from different hospitals can be frustrating. Variability in laboratory results can occur due to differences in testing methods, timing, and even sample handling. If one facility indicates an infection while another does not, it may be beneficial to discuss these results with your healthcare provider. They may recommend further testing, such as a prostate-specific antigen (PSA) test or imaging studies, to gain a clearer understanding of your condition.
As for your current treatment with the antibiotic prescribed by the latest doctor, it is essential to communicate openly with them about your previous treatments and ongoing symptoms. If you feel uncertain about the diagnosis or treatment plan, seeking a second opinion from a urologist specializing in male sexual health may provide additional insights and options.
In addition to medical treatment, lifestyle changes can also play a significant role in managing erectile dysfunction. Regular exercise, a balanced diet, adequate sleep, and stress management techniques can improve overall health and potentially enhance sexual function. Psychological factors, such as anxiety or depression, can also contribute to ED, so addressing these through counseling or therapy may be beneficial.
In summary, erectile dysfunction can be multifactorial, and it is crucial to work closely with your healthcare provider to identify the underlying causes and appropriate treatments. If antibiotics have not resolved your symptoms, further evaluation may be necessary to explore other potential causes and treatment options. Don't hesitate to advocate for your health and seek the care you need to address your concerns effectively.
