Right Rib Pain: Possible Liver or Lung Issues? - Gastroenterology and Hepatology

Right rib cage

Hello, recently I've been experiencing a dull pain in the area between the fifth and sixth ribs on the right side, which occurs occasionally, usually after eating or upon waking up.
In the past, I had sharp pain on the left side of my ribs, and after an examination, the doctor said it was due to poor sleeping posture causing muscle pain, which feels different from this current pain.
I would like to know if it could possibly be related to liver or lung issues? Which specialty should I consult? Thank you.

C, 20~29 year old female. Ask Date: 2021/10/22

Dr. Chen Shidian reply Gastroenterology and Hepatology

Family medicine is more convenient for initial diagnosis and treatment, and referrals to relevant specialties can be made if necessary.

More Info

When it comes to right rib pain, particularly in the area between the fifth and sixth ribs, there are several potential causes to consider, including issues related to the liver, lungs, or even musculoskeletal factors. Given your description of the pain being shallow and occurring after eating or upon waking, it is essential to explore these possibilities further.

Possible Causes of Right Rib Pain
1. Liver Issues: The liver is located in the right upper quadrant of the abdomen, and pain in this area can sometimes be attributed to liver conditions. Given your history as a hepatitis B carrier, it is crucial to monitor liver health. Elevated liver enzymes (SGOT and GPT) can indicate liver inflammation or damage, which may lead to discomfort or pain in the right upper abdomen. Conditions such as fatty liver disease, hepatitis, or even liver cirrhosis can manifest as pain in this area.

2. Gallbladder Problems: The gallbladder is also situated in the right upper quadrant and can cause pain, particularly after eating fatty meals. Gallstones or cholecystitis (inflammation of the gallbladder) can lead to sharp pain that may radiate to the back or shoulder. If you notice that the pain intensifies after consuming greasy foods, this could be a sign of gallbladder issues.

3. Lung Issues: While less common, lung conditions such as pneumonia or pleuritis (inflammation of the lining around the lungs) can cause pain that may be felt in the rib area. If the pain is accompanied by symptoms such as coughing, shortness of breath, or fever, it would be prudent to consider a pulmonary evaluation.

4. Musculoskeletal Pain: As you mentioned experiencing similar pain previously attributed to poor sleeping posture, it is possible that the current discomfort could also be musculoskeletal in nature. Strain or injury to the intercostal muscles (the muscles between the ribs) can cause localized pain that may worsen with certain movements or positions.

Recommendations for Evaluation
Given the variety of potential causes, it is advisable to consult a healthcare professional for a thorough evaluation. Here are some steps you can take:
- Primary Care Physician: Start with your primary care physician, who can perform a physical examination and assess your medical history. They may inquire about the nature of the pain, any associated symptoms, and your overall health status.

- Imaging Studies: Depending on the initial assessment, your doctor may recommend imaging studies such as an ultrasound or CT scan to evaluate the liver, gallbladder, and surrounding structures. These tests can help identify any abnormalities that may be contributing to your pain.

- Referral to Specialists: If liver or gallbladder issues are suspected, your primary care physician may refer you to a gastroenterologist or hepatologist for further evaluation. If lung issues are a concern, a pulmonologist may be the appropriate specialist.

- Lifestyle Modifications: In the meantime, consider making lifestyle changes that may alleviate symptoms. This includes maintaining a healthy diet, avoiding excessive alcohol consumption, managing stress, and ensuring proper sleep posture.

Conclusion
In summary, right rib pain can stem from various sources, including liver, gallbladder, lung, or musculoskeletal issues. Given your medical history and the nature of your symptoms, it is essential to seek medical advice for a comprehensive evaluation. Early diagnosis and appropriate management can significantly improve your quality of life and prevent potential complications. Remember, your health is paramount, and addressing these concerns with a healthcare professional is the best course of action.

Hepatitis rarely causes pain in the lower right quadrant. Your pain is likely 90% due to musculoskeletal pain. The method for examination is quite simple; you should go to the hospital for an ultrasound to rule out the possibility of liver cancer.
