Foreskin Issues: Sensitivity and Short Frenulum Explained - Urology

Glans and foreskin issues?

When I have an erection, the foreskin does not retract on its own, and I have to push it back manually to expose the glans.
Sometimes, the foreskin can swell around the edges overnight.
I previously consulted a doctor who mentioned that my frenulum is shorter, and my glans is very sensitive, which causes me to ejaculate quickly during masturbation.

Hello: You are currently only 15 years old and still in the developmental stage, with "unlimited potential" in the future, so there is no need to worry.
When the foreskin retracts (if it is not returned to its original position), slight swelling the next day is a normal phenomenon (but it is recommended to restore it to its original state after use).
At your age, you may not have much practical experience, so it is normal to ejaculate quickly with stimulation during masturbation.
Please do not overthink or worry about it.

When it comes to issues related to the foreskin, such as sensitivity and a short frenulum, it's important to understand the anatomy and physiological responses involved. The frenulum is a band of tissue located on the underside of the penis that connects the glans (the head) to the shaft. A short frenulum can restrict the movement of the foreskin, making it difficult for it to retract naturally during an erection. This condition is known as frenulum breve.

In your case, the fact that your foreskin does not retract on its own during an erection and requires manual assistance suggests that you may have a tight or short frenulum. This can lead to discomfort and sensitivity, particularly during sexual activities, including masturbation. The sensitivity of the glans can also be heightened due to the lack of exposure, which may contribute to quicker ejaculation during self-stimulation.

The swelling you experience around the foreskin may be due to several factors, including irritation, inflammation, or even an allergic reaction to products used in the genital area. It's essential to monitor this swelling and consult a healthcare provider if it persists or worsens, as it could indicate a more serious condition, such as balanitis (inflammation of the glans) or a potential infection.

In terms of treatment options, there are several approaches you might consider:
1. Gentle Stretching Exercises: If the frenulum is indeed short, gentle stretching exercises may help to gradually increase its length and flexibility. This should be done with care to avoid pain or injury.

2. Topical Treatments: If sensitivity is a significant issue, topical anesthetics may be recommended to reduce sensitivity during sexual activity. However, these should be used under the guidance of a healthcare professional.

3. Surgical Options: In cases where the frenulum is significantly restricting movement and causing discomfort, a minor surgical procedure called frenuloplasty may be performed. This procedure involves lengthening the frenulum to allow for greater mobility of the foreskin.

4. Hygiene Practices: Maintaining proper hygiene is crucial. Ensure that the area is clean and dry, as moisture can lead to irritation and infections. Avoid using harsh soaps or products that may cause allergic reactions.

5. Consultation with a Specialist: If the issues persist or if you have concerns about your sexual health, it may be beneficial to consult a urologist or a sexual health specialist. They can provide a thorough examination and discuss potential treatment options tailored to your specific situation.

In summary, the combination of a short frenulum and heightened sensitivity can lead to challenges during sexual activities. It is essential to address these issues with appropriate medical guidance to ensure both comfort and sexual health. If you experience persistent swelling or discomfort, seeking medical advice is crucial to rule out any underlying conditions and to explore suitable treatment options.
