Waking up startled due to the inability to breathe while asleep?
Hello, doctor.
Recently, I have been experiencing sudden awakenings during sleep due to a sensation of not being able to breathe.
It feels similar to nasal congestion, but I do not actually have nasal blockage.
I can only fall asleep while sitting up, but I wake up with discomfort in my back and lower back.
In the past year, my health check-ups have shown no abnormalities, and there is no family history of heart disease.
I usually exercise for 30 minutes daily, have a normal BMI, and my body fat is around 27-29%.
I often feel unexplained tightness in my back, but I do not snore or experience sleepwalking.
I would like to ask the doctor what potential issues could cause difficulty breathing after falling asleep.

Dr. Jian Shuntian reply Pulmonology
Hello: It is very likely that you have sleep apnea.
Please visit a hospital with a sleep center for a sleep study.
Thank you.
Sincerely, Chien Shun-Tian, Chest Hospital.

Awakening from sleep due to breathing difficulties can be a distressing experience, and it is essential to understand the potential causes behind this phenomenon. Based on your description, several factors could contribute to your symptoms, and I will outline some possible explanations.
1. Sleep Apnea: One of the most common causes of waking up due to breathing difficulties is obstructive sleep apnea (OSA). This condition occurs when the muscles in the throat relax excessively during sleep, leading to a temporary blockage of the airway. Although you mentioned that you do not snore or experience sleepwalking, OSA can still manifest in various ways, including episodes of waking up gasping for air. It is characterized by repeated interruptions in breathing, which can lead to fragmented sleep and daytime fatigue.
2. Nasal Congestion: Even if you do not feel congested, underlying nasal issues such as allergies or sinus problems can lead to a sensation of difficulty breathing during sleep. Nasal congestion can cause you to breathe through your mouth, which may not provide adequate airflow, leading to feelings of suffocation. This can be exacerbated by sleeping positions that may further restrict airflow.
3. Anxiety or Stress: Psychological factors such as anxiety or stress can also contribute to breathing difficulties during sleep. If you are experiencing heightened levels of stress or anxiety, it may manifest as physical symptoms, including a sensation of tightness in the chest or throat, leading to awakening due to perceived breathing difficulties.
4. Gastroesophageal Reflux Disease (GERD): GERD can cause acid to flow back into the esophagus, leading to irritation and discomfort. This can sometimes be felt as a sensation of tightness in the throat or chest, which may wake you from sleep. If you have been experiencing any heartburn or discomfort after eating, this could be a contributing factor.
5. Musculoskeletal Issues: You mentioned experiencing tightness in your back and lower back discomfort. Poor sleeping posture or underlying musculoskeletal issues can lead to discomfort that may affect your breathing. If your body is tense or if you are in an awkward position while sleeping, it could lead to a sensation of restricted breathing.
6. Cardiac Concerns: Although you have ruled out a family history of heart disease and have had normal check-ups, it is still worth considering that certain cardiac conditions can present with symptoms similar to what you are experiencing. Conditions like mitral valve prolapse, which you mentioned, can sometimes lead to palpitations or discomfort that may be perceived as breathing difficulties.
Given these potential causes, it is crucial to consult with a healthcare professional who specializes in sleep medicine or a pulmonologist. They may recommend a sleep study (polysomnography) to monitor your breathing patterns during sleep and assess for conditions like sleep apnea. Additionally, they may conduct further evaluations to rule out other underlying issues, such as anxiety disorders or GERD.
In the meantime, consider maintaining a sleep diary to track your symptoms, including when they occur, their duration, and any associated factors (such as stress levels or dietary habits). This information can be invaluable for your healthcare provider in diagnosing and managing your condition effectively.
Lastly, practicing good sleep hygiene, such as maintaining a consistent sleep schedule, creating a comfortable sleep environment, and managing stress through relaxation techniques, may help improve your overall sleep quality and reduce the frequency of these episodes.
