Chest pain?
Hello, Doctor.
I would like to ask about a recent issue where I occasionally experience sudden sharp pain near my heart or chest (while at rest) that lasts for 2-3 seconds.
It goes away and then returns after a few seconds.
A few months ago, I had an X-ray and echocardiogram at the hospital, and I regularly have blood tests for hypertension, diabetes, and hyperlipidemia, all of which are currently normal.
However, when this sharp pain occurs, I feel inexplicably anxious.
Could you please provide some insight? Thank you.
Chen, 30~39 year old female. Ask Date: 2021/10/28
Dr. Yang Wenda reply Pulmonology
Hello: If recent heart and X-ray examinations are normal, and you experience sudden sharp pain that feels more like nerve pain, it is recommended to manage stress, ensure good sleep quality, and temporarily avoid cold foods, as this may lead to improvement!

More Info
Understanding sudden chest pain is crucial, as it can be a symptom of various underlying conditions, some of which may require immediate medical attention. Based on your description of experiencing sharp, stabbing pain near the heart area that lasts for a few seconds and then subsides, it’s important to consider several potential causes.
Firstly, it's reassuring that you have undergone X-rays, echocardiograms, and regular blood tests for hypertension and other cardiovascular risk factors, and that these results have been normal. This suggests that your heart's structure and function are likely healthy, which is a positive sign. However, the nature of your symptoms warrants further exploration.
Possible Causes of Chest Pain
1. Musculoskeletal Issues: One common cause of chest pain is musculoskeletal strain. This can occur from activities that involve heavy lifting, poor posture, or even prolonged periods of sitting, especially if you work at a computer. The pain from muscle strain can mimic heart-related pain and may be sharp or stabbing.
2. Gastroesophageal Reflux Disease (GERD): Another possibility is GERD, where stomach acid flows back into the esophagus, causing a burning sensation or sharp pain in the chest. This pain can sometimes be mistaken for heart pain, especially if it occurs after eating or when lying down.
3. Anxiety and Panic Attacks: Given that you mentioned feeling tense when the pain occurs, anxiety could be a contributing factor. Anxiety and panic attacks can manifest physically, causing chest pain, palpitations, and a sense of impending doom. This type of pain is often sharp and can come and go.
4. Cardiac Issues: While your previous tests are normal, it’s essential to remain vigilant about cardiac health. Conditions such as angina or even pericarditis (inflammation of the lining around the heart) can cause sharp chest pain. If the pain is accompanied by other symptoms like shortness of breath, sweating, or radiating pain to the arm or jaw, it is crucial to seek immediate medical attention.
5. Pulmonary Issues: Conditions affecting the lungs, such as pleuritis (inflammation of the lining of the lungs) or pulmonary embolism (a blood clot in the lungs), can also cause sharp chest pain. These conditions often present with additional symptoms like difficulty breathing or coughing.
When to Seek Help
You should seek immediate medical attention if you experience any of the following:
- Persistent or worsening chest pain
- Pain that radiates to the arms, neck, back, or jaw
- Shortness of breath, dizziness, or fainting
- Sweating or a feeling of impending doom
- Any new symptoms that are concerning or different from your usual experience
Conclusion
While your previous evaluations are reassuring, the nature of your symptoms suggests that further investigation may be warranted, especially if they persist or worsen. It may be beneficial to consult with a healthcare provider who can perform a thorough assessment, including a detailed history and possibly additional tests, to rule out any serious conditions. Keeping a diary of your symptoms, including their frequency, duration, and any associated factors (like stress or physical activity), can also provide valuable information for your healthcare provider.
In the meantime, consider lifestyle modifications such as stress management techniques, regular physical activity, and maintaining a healthy diet, as these can help alleviate some symptoms and improve overall well-being.
