Left breast mass issue?
Hello doctor, I am 24 years old.
Last year, I discovered a hard lump in my left breast and underwent ultrasound, blood tests, and MRI at the hospital.
The pituitary gland showed no issues, but the doctor mentioned that I have elevated prolactin levels, which is indicative of gynecomastia.
Recently, the hard lump in my left nipple has been painful when pressed.
I would like to know what could be the cause of this.

Dr. Li Zhonghuai reply Surgery
Hello! Are you currently taking any psychiatric medications, stomach medications, diuretics, or antihypertensive medications? If so, adjustments to your medications may be necessary.
Regarding gynecomastia in males, surgical removal may be considered primarily for cosmetic reasons and symptom management.
Given your age, the likelihood of malignancy is extremely low.
Take care! Changhua Hospital cares about you!

When it comes to understanding the causes of a painful lump in the left breast of a young woman, several factors must be considered. At 24 years old, the likelihood of breast cancer is relatively low, but it is essential to be vigilant about any changes in breast tissue. The symptoms you describe, including the presence of a hard lump and tenderness in the left nipple, can be associated with various benign conditions, particularly in younger women.
One of the most common causes of breast lumps in young women is fibrocystic breast changes. This condition is characterized by the presence of cysts and fibrous tissue in the breast, which can lead to pain, tenderness, and the formation of lumps that may fluctuate with the menstrual cycle. Hormonal fluctuations, particularly related to estrogen and progesterone, can exacerbate these symptoms. Since you mentioned that you have elevated prolactin levels, which can lead to conditions like gynecomastia (male breast tissue development), it is possible that hormonal imbalances are contributing to your breast pain and lump formation.
Another potential cause of your symptoms could be a benign tumor, such as a fibroadenoma. Fibroadenomas are solid, non-cancerous lumps that are most commonly found in women in their 20s and 30s. They are usually painless but can sometimes cause discomfort, especially if they are pressing against surrounding tissues or nerves.
In your case, the tenderness associated with the hard lump in your left breast could also indicate inflammation or infection, such as mastitis, particularly if there are any signs of redness, warmth, or discharge. However, since you have undergone imaging studies like ultrasound and MRI, and if these tests did not reveal any significant abnormalities, the likelihood of an infectious process is lower.
It is also important to consider the possibility of a cyst. Breast cysts are fluid-filled sacs that can develop in the breast tissue and may cause discomfort or pain, especially if they become large or if they are located near sensitive areas like the nipple.
Given your history of elevated prolactin levels and the recent development of tenderness in the left nipple, it would be prudent to follow up with a healthcare provider who specializes in breast health. They may recommend further evaluation, such as a targeted ultrasound or even a biopsy if there is any concern about the nature of the lump. Additionally, they may consider adjusting any medications that could be influencing your hormone levels.
In summary, while the presence of a painful lump in the left breast of a young woman can be concerning, many benign conditions can cause similar symptoms. It is crucial to maintain regular follow-ups with your healthcare provider, monitor any changes in your symptoms, and address any concerns you may have regarding your breast health. Early detection and intervention are key to managing any potential issues effectively.
