Irregular menstruation while breastfeeding?
Hello doctor, I am still breastfeeding my baby who is two and a half years old.
My menstrual period returned about a year and a half ago and was regular for the first six months.
However, in the past few months, my cycle has become irregular with periods on July 13, August 17, and September 21.
Now, it is the end of October and I still haven't gotten my period.
We have been using withdrawal method.
Is it possible for me to be pregnant? Thank you 😊

Dr. Lin Manying reply Obstetrics and Gynecology
Ms.
Ming: Hello, during breastfeeding, ovulation can be irregular, and there is still a possibility of pregnancy even with withdrawal method.
It is recommended to take a pregnancy test first.

Breastfeeding and irregular periods can be a common concern for many mothers. When a woman is breastfeeding, especially exclusively, her body produces higher levels of the hormone prolactin, which can suppress ovulation. This hormonal imbalance often leads to irregular menstrual cycles or even the absence of periods altogether, a condition known as lactational amenorrhea.
In your case, it seems that you experienced a regular menstrual cycle for the first six months after your periods returned, but now you are noticing irregularities. This can happen for several reasons. First, breastfeeding frequency can impact your menstrual cycle. If your breastfeeding routine has changed—whether due to the baby eating solids, reducing the number of feedings, or any other factors—this can affect your hormone levels and lead to irregular periods.
Additionally, stress, changes in weight, and overall health can also contribute to menstrual irregularities. If you are experiencing significant stress or have had changes in your lifestyle, these factors can further disrupt your cycle.
Regarding your concern about the possibility of pregnancy, while it is less likely to conceive while breastfeeding, it is still possible. The method of contraception you mentioned, "pulling out" or withdrawal, is not a reliable form of birth control. Pre-ejaculate fluid can contain sperm, and if you are ovulating, there is a chance of pregnancy. Since your periods have become irregular, it may be more challenging to predict ovulation, which increases the risk of unintended pregnancy.
If you are concerned about the possibility of being pregnant, it would be wise to take a home pregnancy test. These tests are generally reliable and can provide you with quick answers. If the test is negative and your periods remain irregular, or if you have other symptoms such as unusual pain or discharge, it would be advisable to consult with a healthcare provider. They can help assess your situation, rule out any underlying issues, and discuss potential contraceptive options if you wish to avoid pregnancy.
In summary, breastfeeding can lead to irregular periods due to hormonal changes, and while the risk of pregnancy is lower, it is not eliminated. If you have concerns about your menstrual cycle or potential pregnancy, consulting with a healthcare professional is the best course of action. They can provide personalized advice based on your health history and current situation.
