To confirm the date of pregnancy?

The last menstrual period was on July 13.
An ultrasound on September 6 showed a gestational age of 7 weeks and 0 days, with a margin of ±12 days (due date April 25 of the following year).
Another ultrasound indicated a gestational age of 7 weeks and 3 days, also with a margin of ±12 days (due date April 22 of the following year).
Intercourse without protection occurred on July 17, 18, and 19.
I would like to confirm which date is more accurate for conception.

M, 30~39 year old female. Ask Date: 2021/11/03

Dr. Huang Zifei reply Obstetrics and Gynecology

Dear M,
Basically, it is not possible to accurately predict which day of unprotected intercourse occurred within three consecutive days using ultrasound, unless it is more than a week apart, which would make it easier to determine.
I apologize for not being able to answer your question.

More Info

To accurately determine your conception date, it is essential to consider several factors, including your menstrual cycle, the timing of ovulation, and the results of any ultrasounds performed during your pregnancy. Based on the information provided, let's break down the details to arrive at a more precise estimation of your conception date.

1. Menstrual Cycle and Ovulation: You mentioned that your last menstrual period (LMP) started on July 13. Typically, ovulation occurs about 14 days after the first day of your last period in a regular cycle. However, since menstrual cycles can vary, especially if they are irregular, it is crucial to consider your specific cycle length. If we assume a standard cycle length of around 28 days, ovulation would likely have occurred around July 27. If your cycle is longer, say 35 days, ovulation could have occurred around August 2.
2. Timing of Sexual Intercourse: You indicated that you had unprotected intercourse on July 17, 18, and 19. If ovulation occurred around July 27, the intercourse on July 17 would be too early for conception, as sperm can survive in the female reproductive tract for up to five days, but not typically longer. The intercourse on July 18 and 19 would be closer to the ovulation window, making these dates more likely candidates for conception.

3. Ultrasound Measurements: You mentioned that an ultrasound on September 6 showed a gestational age (GA) of 7 weeks and 0 days, with a range of ±12 days. This means that the actual conception could have occurred anywhere from 5 weeks to 9 weeks prior to the ultrasound date. If we calculate backwards from September 6, a gestational age of 7 weeks would place the estimated conception date around July 23. The second ultrasound indicated a GA of 7 weeks and 3 days, which would suggest a conception date around July 20.
4. Conclusion: Given the information about your menstrual cycle, the timing of sexual intercourse, and the ultrasound results, it is reasonable to conclude that your conception likely occurred between July 18 and July 23. The intercourse on July 18 and 19 aligns closely with the ovulation period, making these dates the most plausible for conception.

5. Estimating Due Date: Your estimated due date of April 25 is consistent with a conception date around late July. Typically, the due date is calculated by adding 280 days (or 40 weeks) to the first day of your last menstrual period. In your case, if conception occurred around July 20, this aligns well with the due date provided by your healthcare provider.

In summary, while it is challenging to pinpoint the exact day of conception due to the variability in menstrual cycles and ovulation timing, the evidence suggests that your conception likely occurred between July 18 and July 23. For the most accurate assessment, it is always best to consult with your healthcare provider, who can consider all factors and provide personalized guidance based on your medical history and ultrasound findings.
