Medications and urethritis associated with fever?

Hello Dr.
Du, I later went to the hospital for a urinalysis, blood tests, and a urine culture (the results of the culture will be available next week).
The doctor confirmed that I have urethritis and prescribed three medications: Cefixime capsules 100 mg, to take two capsules once, Debaril tablets 500 mg, and Iwell tablets.
After receiving the medication yesterday, I mistakenly thought I was supposed to take one Cefixime capsule every 12 hours, but I realized just before my second dose that I needed to take two capsules.
I have since been taking two capsules on time.
I would like to ask the doctor if this could lead to antibiotic resistance in the bacteria.
Additionally, after my visit to the urologist and taking the medication, I developed a fever of around 38.0 to 38.5 degrees Celsius.
I am hesitant to take antipyretics for fear that it might affect my immune function in fighting the infection.
Before taking the medication, my temperature was around 37.4 to 37.8 degrees Celsius.
Could the sudden increase in temperature be related to the medication? If I have a strong urge to urinate, it feels normal, but towards the end of urination, I feel like something is blocking the flow, and I have to stand for a moment before a little urine is expelled.
Is this related to the urethritis? I would also like to ask the doctor what foods I should avoid during this inflammation, and what foods might be helpful for urethritis.
I have quite a few questions, and I appreciate your patience in answering them.
Thank you.

Mr. Hong, 30~39 year old female. Ask Date: 2021/11/03

Hello: Cefixime capsules 100mg should be taken as two capsules at a time.
I also have Debarilin tablets 500mg and Iwell tablets.
After receiving the medication yesterday, I mistakenly thought I should take one capsule of Cefixime every 12 hours, and I only realized I needed to take two capsules just before the second dose.
Since then, I have been taking two capsules on time.
I would like to ask the doctor if this could cause bacteria to develop resistance? --> No, it will not.
Additionally, after taking the medication following my visit to the urologist yesterday, I developed a fever of around 38.0 to 38.5 degrees Celsius.
I am hesitant to take antipyretics for fear they might affect my immune function and hinder bacterial elimination.
Before taking the medication, my temperature was around 37.4 to 37.8 degrees Celsius.
Doctor, could the sudden increase in temperature be related to the medication? --> No, if you do not seek medical attention, the fever could worsen.
It is still recommended to take antipyretics; if you are completely asymptomatic, you may stop.
If you have very strong urine, it is quite normal, but towards the end of urination, you may feel a sensation of blockage and need to stand for a moment to pass a little more urine.
Is this related to urethral inflammation? --> Yes, it is.
I would also like to ask the doctor what foods should be avoided during this period of inflammation? What should I eat that would be helpful for urethral inflammation? --> It is advisable to avoid spicy foods and alcohol.
Drink plenty of fluids, generally around 2500cc is normal, and your urine should be very light in color.
Do not drink to the point of feeling bloated.
Avoid sexual activity, and please remember to return for a follow-up appointment.
Wishing you good health and a speedy recovery.

More Info

Urethritis, or inflammation of the urethra, can be a distressing condition that often requires careful management, including medication, monitoring of symptoms like fever, and dietary considerations. Based on your situation, let's address your concerns in detail.

Medications and Antibiotic Resistance
You mentioned being prescribed Cefixime, a broad-spectrum antibiotic, along with other medications. It's important to follow the prescribed dosage accurately. Taking two capsules as directed is crucial for ensuring the medication's effectiveness. While there is a concern about antibiotic resistance, taking the medication as prescribed should not inherently lead to resistance, especially if you are not exceeding the recommended dosage. Resistance typically arises from incomplete courses of antibiotics or inappropriate use, such as using antibiotics for viral infections.

Fever Management
Regarding your fever, which has risen to 38.0–38.5°C after starting the medication, it is essential to understand that a mild fever can be a normal response to infection as your body fights off the bacteria. The medications you are taking, particularly antibiotics, may not immediately reduce fever as they work to eliminate the infection. It is generally advisable to manage fever with antipyretics like acetaminophen or ibuprofen, as they can help alleviate discomfort without significantly impairing your immune response. If your fever persists or worsens, it’s crucial to consult your healthcare provider.

Urinary Symptoms
The sensation of having something blocking the urethra, especially towards the end of urination, is indeed a common symptom of urethritis. This can be due to inflammation causing swelling in the urethra, which may lead to a feeling of obstruction. It’s important to stay hydrated, as this can help flush out the urinary tract and may alleviate some symptoms.

Dietary Recommendations
Diet can play a significant role in managing urinary tract issues. Here are some dietary tips that may help:
1. Hydration: Aim to drink plenty of fluids, ideally around 2.5 liters (about 84 ounces) per day, unless otherwise directed by your doctor. This helps dilute the urine and can reduce irritation in the urinary tract.

2. Avoid Irritants: It’s advisable to avoid spicy foods, caffeine, alcohol, and acidic foods (like citrus fruits and tomatoes) as they can irritate the bladder and worsen symptoms.

3. Incorporate Anti-Inflammatory Foods: Foods rich in antioxidants and anti-inflammatory properties can be beneficial. Consider incorporating berries, leafy greens, nuts, and fatty fish into your diet.

4. Probiotics: Foods containing probiotics, such as yogurt, may help maintain a healthy balance of bacteria in the body, which can be beneficial during and after antibiotic treatment.

Conclusion
In summary, while you are managing urethritis with antibiotics, it is vital to adhere to the prescribed regimen, monitor your symptoms closely, and manage fever appropriately. Dietary adjustments can also support your recovery. If you experience worsening symptoms, persistent fever, or any new concerns, please reach out to your healthcare provider for further evaluation and guidance. Your health and comfort are paramount, and timely communication with your doctor is essential for effective treatment.
