Questions about Xin Na tablets for regulating menstruation?

I would like to ask about irregular menstruation.
I have been taking Chlormadinone Acetate and Ethinyl Estradiol (commonly known as oral contraceptive pills) for 6 months.
However, this month I forgot to take the pills once or twice and took them the next day.
During this period, I had unprotected sexual intercourse but did not ejaculate inside.
I finished my last pill on October 31, but my period has not arrived yet (it usually comes within 4 to 5 days after taking the pills in previous months).
I have also experienced breast tenderness.
On October 6, I used a pregnancy test, which showed one line.
I would like to know if there is a possibility of pregnancy.

Ethan, 20~29 year old female. Ask Date: 2021/11/10

Dr. Zhang Kunmin reply Obstetrics and Gynecology

Hello,
Combined oral contraceptives (COCs) must be taken at the same time every day.
Proper adherence can achieve a contraceptive efficacy of 99%.
If a dose is missed and there is no expected withdrawal bleeding during the subsequent pill-free interval, pregnancy should be suspected.
It is recommended to consult a gynecologist for evaluation if menstruation does not occur.

Thank you! Wishing you safety and health.

When it comes to concerns about menstrual irregularities and the potential risks of pregnancy while taking Xinna pills (also known as contraceptive pills), it's important to understand how these medications work and the implications of missed doses.

Xinna pills are a type of hormonal contraceptive that are designed to regulate menstrual cycles and prevent ovulation. They contain synthetic hormones that mimic the natural hormones in a woman's body, which helps to stabilize the menstrual cycle. However, if doses are missed, the effectiveness of the contraceptive can be compromised, leading to potential risks of unintended pregnancy.

In your case, since you mentioned that you have missed a couple of doses and then took them the following day, it is crucial to consider how this may have affected your contraceptive coverage. Generally, if you miss one pill, it is recommended to take it as soon as you remember. If you miss two or more pills, the guidance can vary based on the specific instructions provided with the medication, but it often involves using backup contraception for a period of time.

The fact that you have experienced breast tenderness (referred to as "涨奶" or breast engorgement) could be a sign of hormonal changes in your body, which can occur due to the hormonal fluctuations from the pills or potentially from pregnancy. However, breast tenderness alone is not a definitive indicator of pregnancy.

Regarding your pregnancy test on October 6, if it showed only one line, that typically indicates a negative result, meaning that you are not pregnant at that time. However, it is important to note that pregnancy tests are most accurate when taken after a missed period. Since you mentioned that your period is late, it would be advisable to take another pregnancy test now, as it may provide a clearer answer.

If your period does not arrive soon, and you continue to experience symptoms such as breast tenderness, it would be wise to consult with a healthcare provider. They can provide further evaluation and guidance, including whether a blood test for pregnancy might be necessary, as blood tests can detect pregnancy earlier than home urine tests.

In summary, while there is a possibility of pregnancy due to missed doses of Xinna pills, the negative pregnancy test you took on October 6 suggests that you were not pregnant at that time. Given the irregularities in your menstrual cycle and the symptoms you are experiencing, it is best to follow up with a healthcare professional for personalized advice and to discuss your concerns about your menstrual health and contraceptive use. They can help you determine the best course of action moving forward, whether that involves continuing the current medication, switching to another form of contraception, or addressing any underlying health issues that may be contributing to your menstrual irregularities.
