Post-circumcision care
Hello doctor, I had a circumcision on April 5th of this year, but now it's almost the end of the year and the surgical site is still swollen.
Is there a way to treat this? The swelling has not improved for a long time.
What could be the issue? I seek advice on how to manage this swelling.

Hello: Please return for a follow-up appointment.
Some swelling may be due to poor circulation, while other swelling could be scar tissue, which may take longer to improve.
Wishing you good health.

More Info
Persistent swelling after circumcision can be concerning and may arise from various causes. It is essential to understand the underlying reasons for the prolonged swelling and the appropriate treatment options available.
Causes of Persistent Swelling After Circumcision
1. Normal Healing Process: After circumcision, some swelling is expected as part of the healing process. However, if the swelling persists for several months, it may indicate an underlying issue.
2. Infection: One of the most common causes of prolonged swelling is an infection at the surgical site. Signs of infection may include increased redness, warmth, discharge, or fever. If an infection is present, it typically requires antibiotic treatment.
3. Fluid Accumulation (Seroma or Hematoma): Sometimes, fluid can accumulate in the surgical area, leading to swelling. A seroma is a collection of clear fluid, while a hematoma is a collection of blood. Both conditions may require drainage if they do not resolve on their own.
4. Allergic Reaction: An allergic reaction to sutures, topical medications, or even the antiseptic used during the procedure can cause localized swelling.
5. Scar Tissue Formation: Excessive scar tissue (keloids or hypertrophic scars) can develop after surgery, leading to persistent swelling and discomfort.
6. Underlying Medical Conditions: Conditions such as lymphedema or other systemic issues may contribute to swelling in the genital area.
Treatment Options
1. Observation: If the swelling is mild and there are no signs of infection, your doctor may recommend a period of observation. Swelling can take time to resolve, and in some cases, it may improve on its own.
2. Medication: If an infection is suspected, your healthcare provider may prescribe antibiotics. Anti-inflammatory medications can also help reduce swelling and discomfort.
3. Drainage: If there is a significant accumulation of fluid (seroma or hematoma), your doctor may perform a procedure to drain the fluid. This can relieve pressure and promote healing.
4. Topical Treatments: In cases of allergic reactions or irritation, topical corticosteroids or antihistamines may be recommended to reduce inflammation and swelling.
5. Surgical Intervention: In rare cases, if there is significant scar tissue or other complications, a minor surgical procedure may be necessary to correct the issue.
6. Follow-Up Care: Regular follow-up appointments with your healthcare provider are crucial to monitor the healing process and address any concerns that may arise.
When to Seek Medical Attention
It is essential to consult your healthcare provider if you experience any of the following:
- Increased redness, warmth, or discharge from the surgical site
- Fever or chills
- Severe pain that is not relieved by over-the-counter medications
- Any new or worsening symptoms
In conclusion, persistent swelling after circumcision can result from various factors, and it is crucial to identify the underlying cause to determine the appropriate treatment. If you are experiencing prolonged swelling, I recommend scheduling an appointment with your healthcare provider for a thorough evaluation and tailored treatment plan. They will be able to assess your specific situation and provide the best course of action to promote healing and alleviate any concerns you may have.
