Post-Surgery Concerns: What Does That White Line Mean? - Surgery

Post-fistula surgery

Hello Doctor, I had an imaging done of my fistula before the surgery (https://imgur.com/a/cZpEq9t) and it seems that my fistula extends all the way to the scrotum.
I completed the fistula surgery this Monday, November 15.
After taking a sitz bath today, I took a photo and noticed something strange (https://imgur.com/a/1vtHEjm).
What is that white line above the wound? Is it a remnant of the fistula that the doctor missed? Is there a possibility that I will need another surgery?

James, 30~39 year old female. Ask Date: 2021/11/18

Dr. Ke Fangxu reply Surgery

Due to the low resolution of the second photo, it is difficult to accurately determine whether the white line is a result of suture decomposition or if it is the wound tissue becoming macerated after soaking.
It is recommended that you consult with the surgeon for an examination before further discussion.

After surgery, it's common for patients to have concerns about their healing process, especially when they notice unusual signs such as a white line above the surgical wound. In your case, the white line you observed could be due to several factors, and it's essential to understand what it might indicate.

Firstly, the white line could be the result of the sutures used during your surgery. Absorbable sutures, which are commonly used in surgeries, can sometimes appear white as they begin to dissolve. This is a normal part of the healing process. However, if the sutures are still visible after a significant amount of time, it may indicate that they are not dissolving as expected, or they may be external sutures that need to be removed.

Another possibility is that the white line is related to the healing tissue itself. After surgery, the body forms scar tissue, which can sometimes appear lighter or different in color compared to the surrounding skin. This is particularly true if the area has been subjected to moisture, such as during a sitz bath, which can cause the tissue to swell or change color temporarily.

Regarding your concern about whether this white line indicates a missed fistula or if another surgery is necessary, it is crucial to consult with your surgeon. They can perform a thorough examination to determine if the white line is indeed a remnant of the fistula or if it is simply a part of the normal healing process. If there is any concern that a portion of the fistula was not adequately addressed, your surgeon may recommend further imaging or even a follow-up procedure.

In terms of pain and discomfort, it is not uncommon for surgical sites to experience some level of pain, redness, or swelling as they heal. If the area around the white line is painful or shows signs of infection (such as increased redness, warmth, or discharge), it is essential to seek medical advice promptly. Your healthcare provider can assess whether these symptoms are part of the normal healing process or if they indicate a complication that requires intervention.

In summary, the white line you are observing could be related to the sutures or the healing tissue itself. It is essential to follow up with your surgeon for a proper evaluation and to address any concerns about the healing process. They can provide guidance on whether further treatment is necessary and help ensure that your recovery proceeds smoothly. Remember, open communication with your healthcare provider is key to addressing any post-surgical concerns effectively.
