Interactions Between Birth Control Pills and Antibiotics

Hello Doctor, I had unprotected sexual intercourse with my girlfriend today and later discovered that she has been taking cold medicine containing Leucomycin (250mg), which I found out online is an antibiotic.
However, I did not find any reports indicating that it affects contraceptive efficacy.
She just finished her last dose of the preemptive pill yesterday, and we expect withdrawal bleeding to occur on Tuesday.
Therefore, I would like to ask if this medication will affect the effectiveness of the contraceptive? Is there a need to use emergency contraception, or should we just observe the bleeding situation on Tuesday? Thank you, Doctor.

Niming, 20~29 year old female. Ask Date: 2021/11/20

Hello,
Medications that may interact with oral contraceptives include:
1.
Antibiotics (Penicillin, Cephalosporins, Erythromycin, Sulfonamides, Bactrim, Chloramphenicol) - Doxycycline may reduce the effectiveness of oral contraceptives, as it can decrease the gut flora that aids in absorption, thus lowering the contraceptive effect.

2.
Sedatives (Barbiturates) - These can increase the metabolism of contraceptives, reducing their effectiveness.
3.
Antiepileptic drugs (Primidone, Phenytoin, Carbamazepine) - These also increase the metabolism of contraceptives, leading to decreased effectiveness.
4.
Tuberculosis medications (Rifampicin, Isoniazid).
5.
Antidiabetic medications (Rosiglitazone).
6.
Acetaminophen and Ascorbic acid (Vitamin C) - These may inhibit estrogen metabolism, potentially increasing serum concentrations and the occurrence of side effects.
7.
Oral contraceptives can affect the action of anticoagulants.
8.
Smoking - This can increase the metabolism of contraceptives, reducing their effectiveness.
Combined oral contraceptives (COCs) must be taken at the same time every day; proper use can achieve a 99% effectiveness rate.
It is recommended to check for possible pregnancy if there is no expected withdrawal bleeding during the pill-free interval.

Thank you! Wishing you peace and health.

When it comes to the interaction between antibiotics and birth control, there are several important factors to consider. In your case, you mentioned that your girlfriend took Leucomycin, an antibiotic, after engaging in unprotected sexual intercourse. This raises valid concerns about whether the antibiotic could potentially affect the efficacy of her birth control method.

First and foremost, it is essential to understand that not all antibiotics have the same impact on hormonal contraceptives. Most studies and clinical guidelines indicate that the majority of antibiotics do not significantly reduce the effectiveness of birth control pills. However, there are exceptions, with rifampin and rifabutin being the most notable antibiotics that can decrease the effectiveness of hormonal contraceptives. Leucomycin, on the other hand, is not typically associated with this interaction.
The mechanism by which some antibiotics affect birth control pills is primarily related to the metabolism of hormones in the liver. Some antibiotics can induce liver enzymes that metabolize contraceptive hormones more quickly, thereby reducing their effectiveness. However, Leucomycin does not fall into this category, and there is no substantial evidence to suggest that it would interfere with the effectiveness of birth control pills.

Given that your girlfriend has just completed her course of Leucomycin and is expecting her withdrawal bleed, it is reasonable to monitor her menstrual cycle. If she experiences the expected withdrawal bleeding, it is a good sign that her hormonal contraceptive is still effective. However, if she misses her period or experiences any unusual symptoms, it would be prudent for her to take a pregnancy test to rule out the possibility of pregnancy.

As for whether she should take emergency contraception (the "morning-after pill"), it depends on her current situation. If she is concerned about the possibility of pregnancy and it has been less than 72-120 hours since the unprotected intercourse, she may consider taking emergency contraception as a precaution. However, if she is confident in the effectiveness of her birth control and is experiencing her expected withdrawal bleed, it may not be necessary.

In conclusion, Leucomycin is not known to affect the efficacy of birth control pills significantly. It is essential to observe her menstrual cycle and take appropriate steps if any irregularities occur. If there are any lingering concerns or questions, consulting with a healthcare provider for personalized advice would be beneficial. They can provide further guidance based on her specific health history and current medications.
