Long-Term Use of Inderal and Alpraline: Potential Side Effects Explained - Psychiatry

Long-term use?

Hello Doctor, I would like to ask if long-term use of Inderal and Alpraline can cause any side effects.
Several years ago, I experienced sudden anxiety and increased heart rate for no apparent reason, which led me to seek medical attention.
After listening to my symptoms, the doctor diagnosed me with arrhythmia and nervous system disorder, and prescribed Inderal and Alpraline.
Initially, I took them three times a day, then stabilized to once a day, and eventually stopped taking them altogether.
I occasionally took a dose when symptoms occurred, and after two years of complete symptom improvement, I stopped taking them.
Recently, the symptoms have started again, and I am now taking them three times a day.
Therefore, I would like to inquire about the potential long-term side effects of these medications.
Thank you!

Awei, 40~49 year old female. Ask Date: 2021/11/28

Dr. Qiu Junda reply Psychiatry

Hello.
It seems that these medications can improve your symptoms without significant side effects (such as drowsiness, fatigue, and weakness).
As long as you cooperate with the treatment and do not take excessive or random doses, long-term use should not pose major issues.
These medications will be metabolized by the body over time and will not accumulate, causing adverse effects.
Please rest assured.
Thank you.

Long-term use of Inderal (propranolol) and Alpraline (alprazolam) can indeed lead to various side effects, and it's important to understand these potential risks, especially considering your history of anxiety and heart rate irregularities.

Inderal (Propranolol) is a non-selective beta-blocker primarily used to manage hypertension, anxiety, and certain types of heart conditions. While it can be effective in reducing symptoms of anxiety and controlling heart rate, long-term use may lead to several side effects:
1. Fatigue and Drowsiness: Many patients report feeling tired or lethargic, which can impact daily activities.

2. Cold Extremities: Since propranolol reduces blood flow to the extremities, some individuals may experience cold hands and feet.

3. Bradycardia: This medication can slow the heart rate, which may be problematic for some individuals, especially those with existing heart conditions.

4. Respiratory Issues: In patients with asthma or other respiratory conditions, propranolol can exacerbate symptoms due to its non-selective nature.

5. Mood Changes: Some users report feelings of depression or mood swings, which can be concerning, especially for those with a history of anxiety or depression.

6. Withdrawal Symptoms: If Inderal is discontinued abruptly after long-term use, it may lead to rebound hypertension or increased heart rate.

Alpraline (Alprazolam) is a benzodiazepine used for anxiety and panic disorders. While effective, it carries its own set of risks, particularly with long-term use:
1. Dependence and Tolerance: One of the most significant concerns with alprazolam is the potential for developing a dependence on the medication. Over time, patients may require higher doses to achieve the same effect, leading to a cycle of increased use.

2. Withdrawal Symptoms: Stopping alprazolam suddenly can lead to withdrawal symptoms, including anxiety, insomnia, seizures, and even rebound anxiety, which can be severe.

3. Cognitive Impairment: Long-term use of benzodiazepines has been associated with memory issues and cognitive decline, particularly in older adults.

4. Drowsiness and Sedation: Similar to propranolol, alprazolam can cause significant drowsiness, impacting daily functioning and increasing the risk of accidents.

5. Mood Changes: Some individuals may experience increased anxiety or mood swings, particularly during withdrawal or if the medication is not taken as prescribed.

Given your history of anxiety and heart rate irregularities, it's crucial to have regular follow-ups with your healthcare provider. They can monitor your symptoms and adjust your treatment plan as necessary. If you find that you are relying on these medications more frequently, it may be worth discussing alternative therapies or lifestyle changes that could help manage your symptoms without the risks associated with long-term medication use.

In conclusion, while both Inderal and Alpraline can be effective for managing anxiety and related symptoms, their long-term use should be approached with caution. Regular consultations with your healthcare provider can help mitigate risks and ensure that you are receiving the most appropriate care for your condition. Always consult your doctor before making any changes to your medication regimen.
