The heartbeat fluctuates between fast and slow, and it feels like there are missed beats?
Of course, I have mitral valve prolapse and mild mitral regurgitation, along with ventricular premature contractions for about 20 years.
I used to take 10 mg of antiarrhythmic medication when I felt uncomfortable, but as the frequency of discomfort has increased with age, I now take it two to three times a day in combination with Amiodarone and Sotalol.
Recently, after receiving my second dose of the AZ vaccine, I noticed that my heart rate sometimes drops suddenly from the range of 80-95 down to around 40 or 48, before returning to the normal range.
Occasionally, I experience dizziness and some sharp chest pain (which doesn't last long), but when I went to a nearby clinic for an ECG and also had an emergency ECG and blood tests (including cardiac enzymes and thrombosis), everything came back normal.
I'm unsure of the cause.
I would like to know if further examinations are necessary.

Hello, the myocardial enzymes and thrombus tests are normal, which is reasonable.
However, if you still feel unwell, it is advisable to arrange for a 24-hour Holter monitor for further examination.

Understanding irregular heartbeats, or arrhythmias, is crucial for maintaining cardiovascular health. Irregular heartbeats can manifest in various forms, including premature ventricular contractions (PVCs), atrial fibrillation, and bradycardia, among others. In your case, you have a history of mitral valve prolapse and have experienced symptoms such as occasional dizziness and chest discomfort, particularly after receiving the second dose of the AZ vaccine.
Causes of Irregular Heartbeats
1. Structural Heart Conditions: Conditions like mitral valve prolapse can lead to irregular heartbeats. The valve's improper closure can cause the heart to work harder, potentially leading to arrhythmias.
2. Electrolyte Imbalances: Low levels of potassium, magnesium, or calcium can disrupt the electrical signals in the heart, leading to irregular rhythms.
3. Medications: Certain medications, including those for heart rhythm control, can sometimes cause side effects that lead to irregular heartbeats.
4. Stimulants: Caffeine, nicotine, and certain recreational drugs can increase heart rate and lead to arrhythmias.
5. Stress and Anxiety: Emotional stress can trigger episodes of irregular heartbeats.
6. Post-Vaccination Effects: Some individuals report transient changes in heart rhythm following vaccinations, including the COVID-19 vaccines. This may be due to immune responses or stress reactions.
When to Seek Further Testing
Given your symptoms and history, it is essential to monitor your condition closely. Here are some indicators that suggest you should seek further evaluation:
1. Frequency and Duration of Symptoms: If you experience frequent episodes of bradycardia (heart rate dropping to 40-48 bpm) or if these episodes last longer than a few seconds, further investigation is warranted.
2. Associated Symptoms: Dizziness and chest pain, even if brief, can be concerning, especially if they occur with changes in heart rate.
3. Response to Medication: If your current medication regimen (including rhythm control medications) does not alleviate your symptoms or if you require increasing doses, this could indicate a need for reevaluation.
4. Family History of Heart Disease: If there is a family history of sudden cardiac events or arrhythmias, this may necessitate more aggressive monitoring.
Recommended Next Steps
1. Holter Monitor: A 24-hour Holter monitor can provide a more comprehensive view of your heart's rhythm over time, capturing any irregularities that may not appear during a standard ECG.
2. Electrophysiology Study: If arrhythmias are confirmed and symptomatic, an electrophysiology study may be warranted to assess the heart's electrical system more closely.
3. Echocardiogram: An echocardiogram can evaluate the structure and function of your heart, particularly the mitral valve, to ensure there are no significant changes that could contribute to your symptoms.
4. Lifestyle Modifications: Reducing caffeine intake, managing stress through relaxation techniques, and ensuring adequate hydration can help mitigate symptoms.
5. Regular Follow-Up: Continue to have regular follow-ups with your cardiologist to monitor your condition and adjust treatment as necessary.
In conclusion, while your recent tests have returned normal results, the symptoms you describe warrant further investigation, especially given the changes in your heart rate and associated symptoms. It is essential to maintain open communication with your healthcare provider to ensure that any potential issues are addressed promptly.
