Penile issues
Hello doctor, about four to five years ago, I experienced pain and some bleeding during intercourse with my girlfriend.
This issue didn't occur for the next few years, but about one or two years ago, I had pain again, which went away when I changed positions.
I didn't pay much attention to it.
However, during recent health check-ups, I found traces of blood in my urine, and this time bacteria were detected, while other urine tests were normal.
Due to the pandemic, I haven't seen my girlfriend much, only a couple of times, but when we did have intercourse, it was over quickly.
Recently, I've been relying on masturbation to relieve myself.
Last night, I watched adult films but didn't masturbate, and during the night, I experienced involuntary ejaculation.
In the morning, I felt tightness and soreness in my penis during an erection, which subsided after urinating.
I've had similar experiences before, but I feel a slight swelling in the area between my penis and navel.
I've also been dealing with gastrointestinal bloating, and my doctor prescribed medication for that.
Additionally, I seem to have lost my morning erections, and the force of my urine stream isn't very strong.
It feels like normal urination, though.
Could you please advise if these issues are related, or should I just rest?

Hello: Your urine shows traces of blood, so you should have it checked by a urologist at the hospital.
At 48 years old, you may have prostate issues, so it should be examined as well.
Wishing you good health.

It sounds like you are experiencing a complex set of symptoms that could be related to several underlying issues, including potential infections, psychological factors, or even anatomical concerns. Let's break down your symptoms and provide some insights based on medical knowledge.
Firstly, the pain during intercourse and the presence of blood you experienced years ago could indicate a few possibilities. Conditions such as penile trauma, infections (like sexually transmitted infections), or even issues related to the foreskin (if uncircumcised) could lead to such symptoms. It's important to note that any history of pain or bleeding during sexual activity should be evaluated by a healthcare professional, as it can signify underlying health issues that need to be addressed.
The recent findings of hematuria (blood in urine) and bacterial presence in your urine are significant. Hematuria can arise from various causes, including urinary tract infections (UTIs), kidney stones, or even more serious conditions like tumors. The fact that your urine tests came back normal aside from these findings suggests that while there may not be a widespread infection, localized issues could still be present. It would be prudent to follow up with a urologist who can conduct further evaluations, such as imaging studies or cystoscopy, to rule out any serious conditions.
Your mention of experiencing tightness and discomfort in the penis, especially after an erection, could be related to several factors. One possibility is that you may be experiencing a condition known as Peyronie's disease, which involves the development of fibrous scar tissue inside the penis, leading to curved, painful erections. However, this is just one potential explanation, and a thorough examination by a urologist would be necessary to determine the exact cause.
The absence of morning erections (nocturnal penile tumescence) can also be a sign of underlying issues, including hormonal imbalances or psychological factors such as anxiety or stress. It's not uncommon for men to experience fluctuations in erectile function, especially during periods of stress or when dealing with health concerns. If this is a new development for you, it may be worth discussing with your healthcare provider.
Regarding your urinary concerns, if you feel that the force of your urine stream is weaker than normal, it could indicate a blockage or narrowing in the urinary tract, possibly due to an enlarged prostate or other anatomical issues. Again, this is something that a urologist can help assess through physical examination and possibly imaging studies.
In summary, while some of your symptoms may seem to be related to temporary issues, the combination of pain, hematuria, and changes in urinary function warrants a comprehensive evaluation. It is crucial to consult with a urologist who can provide a thorough examination and appropriate diagnostic tests. They may recommend urine cultures, imaging studies, or even a referral to other specialists if necessary.
In the meantime, it would be beneficial to maintain a healthy lifestyle, manage stress, and avoid any irritants that could exacerbate your symptoms. Keeping a symptom diary, noting when symptoms occur and any potential triggers, can also be helpful for your healthcare provider in determining the best course of action. Remember, early intervention is key to addressing any health concerns effectively.
