Electrosurgery for the removal of facial scars and blemishes?

Hello, Director.
I would like to ask you about the scar issue on my face: I had two areas on my face treated with electrosurgery about a month and a half ago, each approximately the size of a five-cent coin.
Immediately after the procedure, I applied artificial skin, and now the two wounds have healed (though the healing was a bit slow, taking over three weeks before I stopped using the artificial skin).
Currently, the condition of these two areas is that the skin is red with some hyperpigmentation.
I consulted a dermatologist who noted that my wound healing is quite good, and for the redness and hyperpigmentation, they advised me to pay attention to sun protection and prescribed Silian cream to apply morning and night.
Additionally, since I am concerned that these two areas may develop hypertrophic or atrophic scars, I purchased silicone scar gel to apply to these areas after the wounds healed, even before my consultation.
I have two questions for you: 1.
Is it sufficient to apply only the Silian cream and sun protection for the redness and hyperpigmentation of these two areas? 2.
Since the wounds have been healed for a month and a half, I am worried that the scars have not matured yet and that I may develop hypertrophic or atrophic scars in the future.
Therefore, in addition to applying the Silian cream prescribed by the doctor, should I continue using the Fiyide scar gel (silicone + vitamin C) on these two areas? If both can be applied, which one should be applied first: the scar gel or the Silian cream? I would greatly appreciate your advice on these questions.
Thank you.

Dr. Wu Yuxin reply Dermatology

1.
Redness and subsequent hyperpigmentation of the wound are part of the laser treatment process.
2.
It is recommended to apply scar ointment 2-3 times a day, and Silian cream should be used before bedtime.

Managing post-electrosurgery scars can be a concern for many patients, especially when the procedure involves sensitive areas like the face. Based on your description, it seems you are experiencing some redness and hyperpigmentation following your electrosurgery, which is a common occurrence. Let’s address your questions and provide some additional insights into scar management.

1. Redness and Hyperpigmentation Management: The redness and darkening of the skin (hyperpigmentation) you are experiencing is a normal part of the healing process. The skin can take time to return to its normal color, especially after a procedure like electrosurgery. Using the prescribed Silian cream (which likely contains ingredients to promote healing and reduce pigmentation) is a good step. Additionally, sun protection is crucial; UV exposure can worsen hyperpigmentation and delay healing. Therefore, applying a broad-spectrum sunscreen with an SPF of 30 or higher is essential whenever you are exposed to sunlight.

2. Use of Silicone Gel and Vitamin C: Silicone gels are widely recognized for their effectiveness in scar management. They work by hydrating the scar tissue and creating a protective barrier that can help in flattening and softening scars. Vitamin C is also beneficial for skin health and can help in reducing pigmentation. If you are considering using both products, it is generally recommended to apply the silicone gel first, as it needs to penetrate the skin effectively. After the silicone gel has dried, you can apply the Silian cream. This layering can help maximize the benefits of both products.

Additional Tips for Scar Management:
- Patience is Key: Scars can take time to mature. Typically, scars undergo several phases of healing, and it may take up to a year for them to fully mature. During this time, they may change in color, texture, and appearance.

- Moisturization: Keeping the scar area moisturized can help in the healing process. You can use a fragrance-free moisturizer in addition to the silicone gel and Silian cream.

- Avoid Picking or Scratching: It’s important to avoid picking at the scars or the surrounding skin, as this can lead to further irritation and potential scarring.

- Regular Follow-ups: Continue to follow up with your dermatologist or healthcare provider. They can monitor the healing process and suggest additional treatments if necessary, such as laser therapy or chemical peels, if the scars do not improve over time.

- Healthy Lifestyle: Maintaining a healthy diet rich in vitamins and minerals can support skin healing. Hydration is also crucial, so ensure you are drinking enough water.

- Consider Professional Treatments: If you notice that your scars are not improving or if they become raised (hypertrophic or keloid scars), consult your dermatologist about other treatment options, such as corticosteroid injections, laser treatments, or microneedling.

In conclusion, managing post-electrosurgery scars involves a combination of proper skincare, sun protection, and patience. Following your dermatologist's advice and using the recommended products can significantly improve the appearance of your scars over time. Remember that every individual's skin heals differently, so what works for one person may not work for another. Regular check-ins with your healthcare provider will help ensure that you are on the right track to achieving the best possible outcome for your skin.
