How to apply medication for anal fissures?
Hello Doctor: Due to an anal fissure, my doctor prescribed a topical ointment containing hydrocortisone.
However, I read online that steroid-containing ointments should not be used long-term as they can cause skin thinning.
I then switched to using tetracycline and iodine for disinfection.
Is this acceptable? Later, I found out that tetracycline contains antibiotic components, which are also not suitable for long-term use as they may lead to antibiotic resistance in the wound.
This has left me quite confused.
I hope to heal the wound quickly but am unsure how to apply medication correctly without harming my skin.
Currently, I am also doing warm sitz baths.
I would appreciate any advice you can provide.
Thank you.

When dealing with anal fissures, it is crucial to apply medications correctly to promote healing while minimizing the risk of skin damage. Anal fissures are small tears in the lining of the anus, often caused by trauma, constipation, or passing hard stools. They can be quite painful and may lead to further complications if not treated properly.
Understanding the Medications
1. Topical Steroids: Medications like the one you mentioned (樂癒痣) often contain steroids. While they can reduce inflammation and pain, long-term use can lead to skin thinning (atrophy) and other side effects. Therefore, it is generally recommended to use these medications for a limited duration, typically no longer than a few weeks, unless advised otherwise by a healthcare provider.
2. Antibiotic Ointments: You mentioned using a combination of tetracycline and iodine for disinfection. While tetracycline is effective against bacterial infections, prolonged use of antibiotic ointments can lead to antibiotic resistance, which is a significant concern. It's essential to use these medications judiciously and only when necessary.
Recommended Approach for Treatment
1. Consult Your Doctor: Before making any changes to your treatment regimen, it's vital to consult with your healthcare provider. They can provide personalized advice based on the severity of your fissure and your overall health.
2. Use of Medications: If your doctor has prescribed a steroid ointment, use it as directed, but be mindful of the duration. If you are concerned about skin thinning, discuss alternative treatments with your doctor. They may suggest a different topical medication that is less likely to cause skin damage.
3. Sitz Baths: Continue with warm sitz baths, as they can help soothe the area, promote blood flow, and aid in healing. Soaking for 10-15 minutes several times a day can be beneficial.
4. Dietary Changes: To prevent constipation, which can exacerbate fissures, consider increasing your fiber intake through fruits, vegetables, and whole grains. Staying hydrated is also crucial, as it helps soften stools.
5. Gentle Application: When applying any medication, do so gently. Use clean hands or a cotton swab to apply the ointment to avoid further irritation. Avoid vigorous rubbing or scrubbing of the area.
6. Monitor for Side Effects: Keep an eye on the treated area for any signs of adverse reactions, such as increased redness, swelling, or discharge. If you notice any concerning symptoms, contact your healthcare provider promptly.
7. Follow-Up Care: Regular follow-ups with your healthcare provider are essential to monitor the healing process and make any necessary adjustments to your treatment plan.
Conclusion
In summary, while managing anal fissures, it is essential to use medications as prescribed and to be cautious about their long-term use. Combining topical treatments with lifestyle changes, such as dietary adjustments and sitz baths, can significantly enhance healing. Always consult your healthcare provider before making any changes to your treatment plan to ensure the best outcome for your condition.
