Frequent urination issues?

Hello Doctor, I would like to ask about my condition.
I have been experiencing frequent urination for a month.
On the morning of November 6th, after using a toy to stimulate my clitoris and reaching orgasm three times, I started to experience frequent urination, which I suspect may be psychological.
On November 9th, I visited a Western medicine doctor who diagnosed me with urethritis and prescribed a three-day course of antibiotics.
After finishing the antibiotics, I still felt frequent urination, so I went back for another visit.
This time, my urine test showed inflammation again, and the doctor prescribed a week-long course of antibiotics.
After completing that course, I still felt some frequency, so I returned for a follow-up.
This time, the urine test showed no inflammation, but the doctor prescribed another three-day course of antibiotics.
After finishing that, I still felt no improvement.

On November 26th, I decided to see a Traditional Chinese Medicine (TCM) practitioner.
My urine tests were normal.
I had a similar issue last year where Western medicine did not help, but TCM did.
I returned to the TCM practitioner on December 6th, and my urine tests were also normal.
However, this time the duration of TCM treatment has been longer compared to last year, and my symptoms have not improved.

I would like to ask if my normal urine test results mean that it is still possible to have urethritis or cystitis? Since I do not experience pain after urination, could it be overactive bladder or possibly interstitial cystitis? Will medication help with these symptoms?

Hello: Since you have tested positive for a urinary tract infection, the symptoms may be caused by this infection.
Even after a normal urinalysis, it is important to monitor for some time, as there may still be a mild infection.
As for whether it is overactive bladder or interstitial cystitis, observation over a period of time is necessary.
If it is overactive bladder or interstitial cystitis, medication will help improve the condition.
Wishing you good health.

More Info

Frequent urination can be a distressing symptom that affects daily life and can stem from various underlying causes. In your case, it appears that you have been experiencing this symptom for about a month, following a specific incident that may have triggered it. Let's break down the potential causes, treatments, and when to seek further help.

Possible Causes of Frequent Urination
1. Urinary Tract Infection (UTI): Your initial visits to the doctor indicated a urinary tract infection, which is a common cause of frequent urination. UTIs can cause inflammation in the urinary tract, leading to increased urgency and frequency of urination. However, since your urine tests have returned normal after antibiotic treatment, this may no longer be the cause.

2. Psychological Factors: The onset of your symptoms following a specific event (the use of a toy for stimulation) suggests that psychological factors could be at play. Anxiety or stress can manifest physically, leading to symptoms such as frequent urination. This is sometimes referred to as "nervous bladder."
3. Overactive Bladder: This condition is characterized by a sudden urge to urinate that may be difficult to control. It can occur even when the bladder is not full. Symptoms include frequent urination and urgency, but typically do not include pain during urination.

4. Interstitial Cystitis: This is a chronic condition that causes bladder pressure, bladder pain, and sometimes pelvic pain. It can lead to frequent urination, often without the presence of infection. Diagnosis usually requires ruling out other conditions.

5. Other Medical Conditions: Conditions such as diabetes, prostate issues (in males), or neurological disorders can also lead to frequent urination. However, these are less likely given your age and the absence of other symptoms.

Treatment Options
1. Antibiotics: Since you have already been treated with antibiotics multiple times without improvement, it is less likely that a bacterial infection is the cause of your symptoms.

2. Behavioral Modifications: Techniques such as bladder training, which involves gradually increasing the time between urinations, can help manage symptoms of overactive bladder.

3. Medications: If overactive bladder is diagnosed, medications that relax the bladder may be prescribed. These can help reduce urgency and frequency.

4. Psychological Support: If anxiety or psychological factors are contributing to your symptoms, speaking with a mental health professional may be beneficial. Cognitive-behavioral therapy (CBT) can help address anxiety-related symptoms.

5. Alternative Treatments: Since you have also explored traditional Chinese medicine, it may be worth continuing with this approach if you find it helpful. Herbal remedies and acupuncture may provide relief for some individuals.

When to Seek Further Help
If your symptoms persist despite treatment, or if you experience new symptoms such as pain during urination, blood in the urine, or fever, it is important to seek further medical evaluation. A urologist may be able to provide specialized care and conduct tests such as urodynamics to assess bladder function.

Conclusion
In summary, while your symptoms may have initially pointed to a urinary tract infection, the persistence of frequent urination despite treatment suggests that other factors may be involved. It is essential to continue working with healthcare professionals to explore these possibilities, including psychological factors and overactive bladder. Keeping a symptom diary, noting when you experience urgency and any associated activities or stressors, can also provide valuable information for your healthcare provider.
