Is there an issue with benign prostatic hyperplasia (BPH)?
Hello, during my university days, I often felt that urinating was not smooth, and I even hesitated to go to the restroom with classmates because I couldn't urinate.
Sometimes, I would go into the stall when there were people around.
At home, I feel less pressure, but my urine flow is still slow.
Even when I have a strong urge to urinate (like when I hold it in), the flow is not forceful; it’s more like a faucet opened two-thirds of the way.
Towards the end of urination, I need to squeeze my anus to expel the last drops of urine, and I also have to press on the perineum to push out residual urine.
Could this be caused by benign prostatic hyperplasia? I usually masturbate twice a week; could that be related? Thank you.

Hello: It is generally recommended for men to have a prostate examination by a urologist at least after the age of 40 to check for any other issues.
Wishing you good health.

More Info
Prostate enlargement, also known as benign prostatic hyperplasia (BPH), is a common condition that can significantly affect urinary function, especially in older men. Your symptoms, such as a weak urine stream, difficulty starting urination, and the sensation of incomplete bladder emptying, are indeed consistent with BPH. However, it’s essential to consider other potential causes and factors that could contribute to your urinary issues.
Understanding Prostate Enlargement and Urination Issues
1. Prostate Anatomy and Function: The prostate gland surrounds the urethra, the tube through which urine exits the body. As the prostate enlarges, it can compress the urethra, leading to various urinary symptoms. This compression can cause a weak stream, difficulty initiating urination, and a feeling of incomplete emptying.
2. Symptoms of BPH: Common symptoms associated with BPH include:
- Frequent urination, especially at night (nocturia)
- Urgency to urinate
- Difficulty starting urination
- Weak or interrupted urine stream
- Dribbling at the end of urination
- Feeling of incomplete bladder emptying
3. Age and Risk Factors: While BPH is more common in older men, younger individuals can experience similar symptoms due to other factors, such as anxiety, pelvic floor dysfunction, or even urethral stricture. Your experience in college, where you felt pressure and anxiety about urination in social situations, could have contributed to your symptoms.
Potential Causes Beyond BPH
1. Psychological Factors: Anxiety and stress can significantly impact urinary function. If you felt nervous or pressured in social settings, this could lead to a psychological barrier that affects your ability to urinate comfortably.
2. Pelvic Floor Dysfunction: The pelvic floor muscles play a crucial role in urination. If these muscles are tense or not functioning correctly, they can contribute to urinary difficulties. Techniques such as pelvic floor exercises (Kegel exercises) may help improve muscle control.
3. Urethral Stricture or Infection: Conditions such as urethral stricture (narrowing of the urethra) or urinary tract infections can also lead to similar urinary symptoms. It may be worthwhile to consult a urologist for further evaluation.
The Role of Masturbation
Masturbation, in general, is a normal sexual activity and is not typically associated with urinary issues. However, if you are experiencing discomfort or pain during or after ejaculation, it might be worth discussing with a healthcare provider, as it could indicate an underlying issue.
Recommendations
1. Consult a Urologist: Given your symptoms, it would be beneficial to consult a urologist. They can perform a thorough evaluation, including a physical examination, urinalysis, and possibly imaging studies, to determine if BPH or another condition is causing your symptoms.
2. Lifestyle Modifications: Staying hydrated, avoiding caffeine and alcohol, and practicing relaxation techniques may help alleviate some urinary symptoms. Additionally, pelvic floor exercises can strengthen the muscles involved in urination.
3. Follow-Up: If diagnosed with BPH, treatment options range from lifestyle changes and medications to more invasive procedures, depending on the severity of your symptoms and their impact on your quality of life.
In conclusion, while prostate enlargement could be a contributing factor to your urinary issues, it’s essential to consider a comprehensive evaluation to rule out other potential causes. Seeking professional medical advice will provide you with the best course of action tailored to your specific situation.
