Is it possible to become pregnant?

My menstrual cycle is about 25-26 days.
This time, my period started on December 12 and ended around December 20.
I was lying on my back when my boyfriend ejaculated on my back, close to my buttocks.
However, he immediately wiped it off with a tissue, and I'm not sure if any sperm got into my anal area.
After that, I also took a shower right away.

1.
What is the likelihood of getting pregnant in this situation?
2.
Is there a need to take emergency contraception?

Kk, 30~39 year old female. Ask Date: 2021/12/21

Dr. Zhang Kunmin reply Obstetrics and Gynecology

Hello, the questioner: As long as there are active sperm in the vagina during the ovulation period, regardless of the method of intercourse, there is still a possibility of pregnancy.
It is recommended that you take appropriate contraceptive measures.
You may consider using emergency contraception; however, the effectiveness of emergency contraception is not 100%, with a failure rate of about 3%.
If your period is more than a week late, it is advisable to check for pregnancy.
Thank you! Wishing you peace and health.

More Info

When it comes to the possibility of getting pregnant from ejaculation on your back, the chances are generally low, but there are some important considerations to keep in mind.
1. Pregnancy Risk: The likelihood of becoming pregnant from semen that is ejaculated on your back is minimal, especially if it does not come into direct contact with the vaginal opening. Sperm needs to travel through the vaginal canal to reach the uterus and fertilize an egg. However, if any sperm were to inadvertently make its way into the vaginal area, there is a potential risk, albeit small. The fact that your boyfriend immediately wiped off the semen and you washed afterward further reduces the chances of sperm entering the vagina.

2. Emergency Contraception: Given the low risk of pregnancy in this scenario, taking emergency contraception (like Plan B or Ella) may not be necessary. However, if you are feeling anxious about the possibility of pregnancy, you could consider taking it as a precaution. Emergency contraceptive pills are most effective when taken within 72 hours after unprotected intercourse, but some types can be effective up to 5 days later. It’s important to note that these pills are not intended for regular use and should only be used in emergencies.

Additional Considerations:
- Understanding Your Cycle: You mentioned that your menstrual cycle is about 25-26 days. If you are tracking your ovulation, it’s important to note that ovulation typically occurs around 14 days before your next period. If your cycle is regular, you would likely be less fertile right after your period, which is when you described the incident occurring.
- Sperm Viability: Sperm can live for a short time outside the body, but they typically do not survive long without the right conditions. The vaginal environment is necessary for sperm to remain viable for fertilization.
- Consulting a Healthcare Provider: If you have any doubts or concerns about your situation, it’s always best to consult with a healthcare provider. They can provide personalized advice based on your health history and current circumstances.

- Future Prevention: If you are sexually active and wish to avoid unintended pregnancies, consider discussing long-term contraceptive options with your healthcare provider. There are many effective methods available, including hormonal birth control, intrauterine devices (IUDs), and barrier methods.

In summary, while the risk of pregnancy from ejaculation on your back is low, it’s understandable to feel concerned. If you are anxious about the possibility of pregnancy, taking emergency contraception can be a consideration, but it may not be necessary given the circumstances you described. Always feel free to reach out to a healthcare professional for guidance tailored to your specific situation.
