The epididymis becomes firm while lying down?
I have been experiencing epididymitis for a while, approximately from the end of July until now.
It has been fluctuating, and recently it has relapsed.
However, it is strange that when I lie down, the epididymis and spermatic cord become hard and painful.
I would like to inquire why this happens.
I have recently gone to get medication, but I still cannot find a cure.
Thank you.

Epididymitis is an inflammation of the epididymis, which is a coiled tube located at the back of the testicle that stores and matures sperm. This condition can be caused by various factors, including bacterial infections, sexually transmitted infections, or even non-infectious causes such as trauma or autoimmune disorders. Your experience of pain and hardness in the epididymis and spermatic cord while lying down can be attributed to several physiological and anatomical factors.
When you lie down, the position of your body can affect the blood flow and pressure in the pelvic region. The epididymis and spermatic cord are sensitive structures, and changes in blood flow can lead to increased pressure and discomfort. Additionally, lying down may exacerbate the inflammation in the epididymis, leading to a sensation of hardness and pain. This is because the inflammatory process can cause swelling and increased vascularity in the area, which may be more pronounced when you are in a horizontal position.
Another possibility is that lying down may alter the way you perceive pain. When you are upright, gravity can help relieve some pressure on the pelvic structures. However, when lying down, the lack of gravitational pull may lead to a pooling of fluids or increased tension in the area, resulting in heightened discomfort.
As for the recurring nature of your epididymitis, it is essential to consider that some cases may not respond fully to initial treatments. Antibiotics are typically prescribed for bacterial infections, but if the underlying cause is not effectively addressed, symptoms may persist or recur. Factors such as incomplete treatment, antibiotic resistance, or the presence of a chronic infection can contribute to this issue. Additionally, if there are anatomical abnormalities, such as a varicocele or hydrocele, these may need to be evaluated and treated to prevent recurrence.
To manage your symptoms and seek a more permanent solution, it is advisable to follow up with a healthcare provider, preferably a urologist. They may recommend further diagnostic tests, such as an ultrasound, to assess the epididymis and surrounding structures for any abnormalities. Depending on the findings, treatment options may include a longer course of antibiotics, anti-inflammatory medications, or even surgical intervention if necessary.
In the meantime, you can take some self-care measures to alleviate discomfort. Applying ice packs to the affected area can help reduce swelling and pain. Additionally, wearing supportive underwear may provide comfort and reduce strain on the epididymis and spermatic cord.
In summary, the pain and hardness you experience while lying down may be due to changes in blood flow, pressure, and inflammation in the epididymis. Recurring symptoms may indicate that the underlying cause has not been fully addressed, and a thorough evaluation by a urologist is recommended for effective management.
