Stool shape
Hello Doctor: I would like to ask, if during bowel movements my stool is formed but accompanied by irregular small pieces (with a feeling of incomplete evacuation) and this has persisted for 2 to 3 years, could it be due to the presence of polyps in the colon? Additionally, I had two episodes of black stools a few weeks ago, although I haven't experienced it since, I am still quite worried.
Should I consider getting a colonoscopy?

Dr. Cai Anshun reply Gastroenterology and Hepatology
Hello, the shape of your stool can be influenced by your diet and is not necessarily caused by polyps.
It is recommended to consult a gastroenterologist, who can determine whether a colonoscopy is necessary.

When it comes to understanding stool shape and its potential implications for colon health, it's essential to consider various factors that can influence bowel movements. The shape, consistency, and frequency of stools can provide valuable insights into gastrointestinal health, and any significant changes should be taken seriously.
Firstly, the presence of irregularly shaped stools, particularly those that are thin or have a "pencil-like" appearance, can be indicative of several conditions. One common concern is the presence of polyps or other growths in the colon, which can narrow the passage through which stool passes. Polyps are abnormal tissue growths that can develop on the lining of the colon and may vary in size. While many polyps are benign, some can develop into colorectal cancer over time. Therefore, if you have been experiencing these symptoms consistently for two to three years, it is prudent to consult a healthcare professional for further evaluation.
The fact that you have experienced black stools on two occasions is also noteworthy. Black stools can be a sign of bleeding in the upper gastrointestinal tract, which can occur due to various reasons, including ulcers, gastritis, or even certain medications and supplements (such as iron). While the black stools have since resolved, this symptom, combined with the changes in stool shape, warrants further investigation.
Given your concerns and the duration of your symptoms, it would be advisable to schedule a colonoscopy. A colonoscopy is a diagnostic procedure that allows a physician to visualize the interior of the colon and rectum using a flexible tube with a camera. This procedure can help identify any abnormalities, such as polyps, tumors, or inflammation, and can also allow for the removal of polyps during the same visit if necessary.
Additionally, it is important to consider your overall gastrointestinal health. If you have a history of gastrointestinal issues, such as irritable bowel syndrome (IBS), which you mentioned in previous discussions, this could also contribute to changes in stool consistency and shape. IBS can lead to symptoms such as diarrhea, constipation, or alternating between the two, and can sometimes cause a sensation of incomplete evacuation.
In summary, the combination of irregular stool shape, a prolonged duration of symptoms, and the occurrence of black stools suggests that a thorough evaluation is warranted. A colonoscopy is a safe and effective way to assess your colon health and rule out any serious conditions. It is always better to err on the side of caution when it comes to gastrointestinal symptoms, especially if they persist over time. Please consult with a gastroenterologist to discuss your symptoms and the need for further diagnostic testing. Taking proactive steps towards your health is crucial, and early detection can significantly improve outcomes if any issues are found.
