Lower abdominal discomfort after sexual intercourse?

Hello doctor, after having sexual intercourse with my boyfriend yesterday, I started feeling a dull ache in my lower abdomen today.
It feels uncomfortable when I stand up and walk around, but it doesn't hurt when I'm sitting down.
There's also a strange sensation of heaviness and a slight feeling of nausea.
I looked up information about corpus luteum rupture, which usually involves severe pain, but I don't have that intense pain.
What could be the possible reasons? Thank you.

Yu, 10~19 year old female. Ask Date: 2021/12/27

Dr. Cao Guotao reply Obstetrics and Gynecology

Hello, it could also be inflammation.
If you still feel unwell, it is advisable to see a gynecologist.
Wishing you good health.

Experiencing lower abdominal discomfort after sexual activity can be concerning, and it's important to understand the potential causes and implications. Based on your description, it seems you are experiencing a combination of dull pain, a feeling of heaviness, and some nausea. While you mentioned that you do not have severe pain, there are several factors that could contribute to your symptoms.

1. Pelvic Congestion: After sexual activity, especially if there was significant arousal or prolonged foreplay, blood flow to the pelvic area increases. This can lead to a sensation of fullness or pressure in the lower abdomen, which may feel uncomfortable or even painful. This condition is often referred to as pelvic congestion syndrome, where the veins in the pelvic region become engorged with blood, leading to discomfort.

2. Muscle Strain: Engaging in sexual activity can sometimes lead to muscle strain in the pelvic floor or abdominal muscles. If you were in positions that required significant effort or if you were tense during the activity, this could lead to soreness in the lower abdomen. This discomfort may be exacerbated by movements such as standing up or walking.

3. Ovulation Pain (Mittelschmerz): If you are in the middle of your menstrual cycle, you might be experiencing ovulation pain, known as mittelschmerz. This can cause a one-sided, dull ache in the lower abdomen and can sometimes be mistaken for discomfort related to sexual activity.

4. Infection: While you mentioned that you do not have severe pain, it is still possible that you could be experiencing a mild urinary tract infection (UTI) or pelvic inflammatory disease (PID). These conditions can cause discomfort in the lower abdomen, especially after sexual activity. Symptoms may include a feeling of pressure, discomfort during urination, or unusual discharge.

5. Cysts or Other Ovarian Issues: Ovarian cysts can sometimes cause discomfort, especially if they rupture. While you noted that you do not have severe pain, small cysts can lead to mild discomfort or a feeling of fullness in the abdomen.

6. Psychological Factors: Anxiety or tension during sexual activity can lead to muscle tightness, which may contribute to discomfort afterward. If you were feeling anxious or tense, this could manifest as physical discomfort.

Given your symptoms, it is advisable to monitor them closely. If the discomfort persists, worsens, or is accompanied by other symptoms such as fever, severe pain, or changes in your menstrual cycle, it would be prudent to seek medical attention. A healthcare provider can perform a thorough evaluation, which may include a pelvic exam, ultrasound, or other diagnostic tests to determine the underlying cause of your discomfort.

In the meantime, consider practicing relaxation techniques, such as deep breathing or gentle stretching, to alleviate muscle tension. Staying hydrated and avoiding any activities that exacerbate the discomfort may also help. If you suspect that your symptoms could be related to an infection or other medical condition, do not hesitate to reach out to a healthcare professional for guidance.
