Discomfort in the right upper abdomen has been evaluated?
I have been experiencing a persistent sensation of heat in the upper right abdomen (similar to the feeling of having a pain relief patch applied) for several months.
It is not painful, but it is distinctly uncomfortable in the area just above the navel and slightly below the rib cage.
This has been ongoing since November, with symptoms occurring daily, particularly during the day when I am active.
I find it more comfortable to lie flat at night.
Sometimes, resting while sitting alleviates the discomfort.
I have undergone several tests, including ultrasound, colonoscopy, and gastroscopy, which revealed mild gastritis and mild gastroesophageal reflux near the duodenum, but the symptoms have not improved with medication.
Earlier this year, I experienced a significant discomfort and went to the emergency room, where I had an electrocardiogram, abdominal X-ray, urinalysis, and blood tests.
The only abnormal finding was a slightly elevated total bilirubin level of about 1.1 mg/dL, with no other issues noted.
I later had an abdominal CT scan, which also reported no abnormalities.
My bowel movements have been normal and appear golden-yellow.
What advanced tests can I consider beyond those I have already undergone? Thank you.

More Info
Persistent upper right abdominal discomfort can be quite challenging to diagnose, especially when initial tests such as ultrasound, colonoscopy, and gastroscopy have not revealed any significant abnormalities. Given your symptoms and the results of your previous examinations, it is essential to consider a few potential next steps for diagnosis and management.
Understanding Your Symptoms
The discomfort you describe, characterized by a persistent sensation of heat or discomfort in the upper right abdomen, particularly under the rib cage, can be attributed to several potential causes. Since you mentioned that the discomfort is more pronounced during the day and alleviates when lying down, it could be related to gastrointestinal issues, musculoskeletal problems, or even referred pain from other organs.
Previous Investigations
1. Ultrasound: This imaging technique is excellent for assessing gallbladder issues, liver conditions, and other abdominal organs. Since your ultrasound did not show any significant abnormalities, gallstones or acute cholecystitis may be less likely.
2. Endoscopy (Gastroscopy and Colonoscopy): These procedures help visualize the upper and lower gastrointestinal tracts. The findings of mild gastritis and gastroesophageal reflux disease (GERD) could explain some discomfort, but since treatment has not alleviated your symptoms, further investigation is warranted.
3. CT Scan: A CT scan of the abdomen is a comprehensive imaging study that can help identify issues such as tumors, abscesses, or other structural abnormalities. Since your CT scan did not reveal any significant findings, this is reassuring but does not completely rule out all potential causes.
Next Steps for Diagnosis
Given that you have already undergone several tests, here are some additional steps you might consider:
1. MRI of the Abdomen: An MRI can provide more detailed images of soft tissues compared to a CT scan. It may help identify issues related to the liver, pancreas, or other abdominal organs that were not clearly visible on the CT scan.
2. HIDA Scan: If gallbladder dysfunction is suspected, a HIDA scan (hepatobiliary iminodiacetic acid scan) can assess the function of the gallbladder and help identify any issues related to bile flow.
3. Esophageal pH Monitoring: Since you have symptoms consistent with GERD, this test can help determine if acid reflux is contributing to your discomfort. It measures the amount of acid in the esophagus over a 24-hour period.
4. Manometry: This test evaluates the function of the esophagus and can help diagnose motility disorders that might contribute to your symptoms.
5. Referral to a Specialist: If your symptoms persist despite these investigations, consider consulting a gastroenterologist or a pain specialist. They can provide a more focused evaluation and may suggest additional tests or treatments based on their expertise.
6. Consider Functional Disorders: Sometimes, symptoms can arise from functional gastrointestinal disorders, such as irritable bowel syndrome (IBS) or functional dyspepsia. These conditions can be challenging to diagnose but may require a different approach to management, including dietary changes, medications, or stress management techniques.
Conclusion
Persistent abdominal discomfort can be multifactorial, and while initial tests may not reveal a clear cause, it is essential to continue exploring potential underlying issues. Collaborating closely with your healthcare provider and possibly seeking a second opinion or referral to a specialist can help you navigate this complex situation. Keep a detailed symptom diary, noting when symptoms occur, their intensity, and any potential triggers, as this information can be invaluable for your healthcare team in determining the next steps for diagnosis and treatment.
