Post-circumcision wound issues and yellowing pain at the meatus?
Hello Doctor: I underwent traditional circumcision surgery on December 29 of last year.
However, I mistakenly returned for a follow-up appointment a week later, where the tighter bandages from the surgery were removed for the first time.
After the bandages were taken off, there was still some bleeding.
I would like to know if this significantly affects the healing of the wound.
There is no swelling around the area, but there is a small bump at the glans where the ligature is tied, and the wound is still painful.
Currently, I am simply changing the bandages myself (the bandages are stained with blood).
I am a bit worried about wound care and hygiene since I will be enlisting next Thursday.
Additionally, it seems that due to the previous tight bandaging and some urinary retention, the opening of the urethra has turned slightly yellow.
Today, I also experienced some difficulty urinating, with a bit of pain, as it took a moment before I could urinate.
I would like to ask if this indicates a possible infection or inflammation.

Hello: I underwent a traditional circumcision on December 29 of last year, but due to a misunderstanding about my follow-up appointment, I returned a week later to have the tighter dressing removed during my first dressing change.
After removing it, there was still some bleeding.

--> This is not a major concern; it is common to have some oozing during the first dressing change, and there should not be significant swelling.
There may be a small bump at the glans due to the sutures, and some pain at the wound site is expected.
I am currently changing the dressing myself (the dressing is stained with blood).
I am a bit worried about wound care and hygiene since I will be enlisting next Thursday.
Additionally, it seems that the previous tight dressing and some urinary retention may have caused some yellowing at the meatus, and today I experienced some difficulty urinating, with a bit of pain.
I would like to know if this indicates an infection or inflammation.
--> A urinalysis is needed to determine that; in the meantime, drink plenty of water.

Post-circumcision complications can indeed be a concern, particularly regarding wound healing and urinary issues. It is essential to address these concerns promptly to prevent further complications.
Firstly, regarding the wound healing aspect, it is not uncommon for patients to experience some bleeding after a circumcision, especially if the dressing was removed later than recommended. The healing process can be affected by several factors, including the tightness of the dressing, the presence of any infection, and the overall health of the individual. If the dressing was too tight, it could have restricted blood flow to the area, potentially leading to complications such as necrosis or delayed healing. However, the absence of significant swelling is a positive sign, indicating that there may not be severe complications at this stage.
The small bump you mentioned at the glans (the head of the penis) could be a normal part of the healing process, such as granulation tissue, which is a sign of healing. However, if it continues to be painful or grows larger, it would be wise to consult your healthcare provider for further evaluation.
As for the urinary concerns, experiencing pain during urination and a sensation of difficulty in starting the flow can indicate a few potential issues. One possibility is that the tight dressing may have caused some irritation or inflammation in the urethra, leading to discomfort. Additionally, if there is any residual blood or scabbing in the area, it could contribute to these symptoms. The yellowing you observed at the meatus (the opening of the urethra) could suggest some irritation or even the beginning of an infection, especially if accompanied by pain.
Infections post-circumcision can occur, and symptoms may include increased pain, swelling, redness, or discharge from the wound. If you notice any of these signs, it is crucial to seek medical attention promptly. Infections can lead to more severe complications if left untreated, including abscess formation or systemic infection.
For wound care, it is essential to keep the area clean and dry. You should follow your healthcare provider's instructions regarding dressing changes. If the dressing is sticking to the wound, you can moisten it with saline or sterile water to help ease the removal process. Avoid using any creams or ointments unless specifically prescribed by your doctor, as these can sometimes trap moisture and lead to infection.
Given that you are about to enter military service, it is crucial to ensure that your wound is healing properly before you proceed. If you have not already done so, I strongly recommend contacting your surgeon or healthcare provider to discuss your symptoms and concerns. They may want to assess the wound and possibly provide additional treatment or guidance to ensure that you are fit for service.
In summary, while some bleeding and discomfort can be normal after a circumcision, persistent pain, difficulty urinating, and signs of infection warrant immediate medical attention. Proper wound care and hygiene are critical to facilitate healing and prevent complications. Always err on the side of caution and consult with your healthcare provider if you have any doubts or concerns about your recovery.
