Super long-term insertion of contraceptive devices?
Hello doctor, I have been using the Mirena IUD for over a year.
Last week, I was unable to insert it on time due to being on an outer island and exceeded the insertion window (after my menstrual cycle).
It was originally scheduled for Tuesday at 12:00 PM, but I ended up inserting it on Friday at 12:00 AM.
Then, the following Tuesday at 11:00 PM, the IUD accidentally fell out.
Do I need to take emergency contraception? If I take it, will it affect the efficacy of the IUD or my menstrual cycle? Additionally, I would like to know if I need to take a break from using the IUD to allow my body to rest, and whether this will affect my future fertility.
Thank you.
Jane, 20~29 year old female. Ask Date: 2022/01/19

Hello, based on your description, since the contraceptive effect may be reduced, you can consider using emergency contraception as a remedy.
The effects of emergency contraception on menstruation can vary; it may cause your period to come earlier or later.
Its contraceptive effectiveness is not 100%, with a failure rate of about 3%.
It is recommended that if your period does not arrive on time, you should check for pregnancy.
If you are using an intrauterine device (IUD), it is best to have it evaluated by a physician annually.
Thank you! Wishing you peace and health.

When it comes to understanding the timing of IUD insertion and the need for emergency contraception, there are several important factors to consider. Based on your situation, it seems you have been using the Mirena IUD (舞悠) for over a year, and you experienced a delay in its insertion. You also mentioned that the IUD fell out shortly after insertion. Let's break down your concerns:
1. Timing of IUD Insertion: Ideally, an IUD should be inserted during the menstrual cycle, specifically during the first seven days of your period. This timing helps ensure that you are not pregnant at the time of insertion. Since you mentioned that your insertion was delayed from Tuesday to Friday, it is crucial to consider whether you had unprotected intercourse during that time. If you had sex after the expected insertion date but before the actual insertion, there is a risk of pregnancy, and emergency contraception may be warranted.
2. Emergency Contraception: If the IUD was inserted correctly and you had unprotected intercourse after the insertion, you generally do not need emergency contraception, as the IUD is effective immediately. However, if the IUD fell out shortly after insertion, and you had unprotected intercourse during that time, it is advisable to consult with your healthcare provider about taking emergency contraception. Taking emergency contraception should not affect the efficacy of the IUD once it is properly in place, but it is essential to follow up with your provider to ensure everything is functioning as it should.
3. Impact on Menstrual Cycle and Fertility: Regarding your question about whether you need to take a break from the IUD to allow your body to rest, it is generally not necessary to stop using the IUD for rest periods. The IUD is designed to provide long-term contraception without significant interruptions. However, if you experience any adverse effects or have concerns about your menstrual cycle, it is best to discuss these with your healthcare provider. They can provide personalized advice based on your medical history and current health status.
4. Future Fertility: Using an IUD does not typically affect future fertility. Once the IUD is removed, most women can conceive immediately. If you have concerns about your fertility or the impact of the IUD on your reproductive health, it is essential to have an open dialogue with your healthcare provider. They can offer guidance and reassurance based on your individual circumstances.
In summary, if you are unsure about the need for emergency contraception due to the timing of your IUD insertion and its subsequent displacement, it is crucial to consult with your healthcare provider. They can assess your situation, provide appropriate recommendations, and ensure that you have the necessary support for your reproductive health. Always prioritize open communication with your healthcare team to address any concerns you may have regarding contraception and fertility.
