Irregular menstrual cycle?

Doctor, I apologize for the interruption.
I would like to ask about my sexual activity before August, which was three years ago.
After having intercourse at the end of August, my menstrual periods were normal until October, when they became irregular, occurring every two to three weeks.
Then, my periods suddenly became abnormal, with only a little bleeding on the first day and very little or none on the second and third days.
However, I noticed some blood in my urine.
By the fourth day, my period gradually returned to normal.
In November, my menstrual cycle was very irregular, but it was normal in December, although the blood flow was quite heavy.
In January, I started experiencing the same issues as in October.
I have taken a pregnancy test, and it was negative.
What could be the cause of this situation?

En, 30~39 year old female. Ask Date: 2022/01/21

Hello, the factors that can affect the menstrual cycle or cause intermenstrual bleeding are numerous, including age, environment, lifestyle, stress, emotional instability, weight changes, medications, emergency contraceptive pills, diet, polycystic ovary syndrome, uterine or cervical lesions, infections, ovulation, pregnancy, and breastfeeding, among others.
It is advisable to consult a gynecologist if symptoms persist or worsen to determine if there is a hormonal imbalance or other underlying causes.
Thank you! Wishing you peace and health, Dr.
Zhang.

Irregular menstrual cycles can be a source of concern for many women, and understanding the underlying causes is crucial for addressing any potential health issues. Based on your description, it seems that your menstrual cycle has experienced significant fluctuations over the past few months, which can be attributed to various factors.

Firstly, it is essential to recognize that menstrual cycles can be influenced by a multitude of factors, including hormonal imbalances, stress, lifestyle changes, and underlying medical conditions. Hormonal fluctuations are often the primary culprits behind irregular periods. Conditions such as polycystic ovary syndrome (PCOS), thyroid disorders, and even perimenopause can lead to changes in menstrual regularity. In your case, since you mentioned that your menstrual cycle was normal for a period before becoming irregular, it might be worth considering if any recent changes in your lifestyle, stress levels, or health status could have contributed to this shift.

Stress, both physical and emotional, can significantly impact the menstrual cycle. High levels of stress can lead to the release of cortisol, a hormone that can interfere with the normal functioning of the reproductive hormones, resulting in irregular periods. Additionally, significant weight changes, whether loss or gain, can also disrupt hormonal balance and lead to irregular cycles.

Another important aspect to consider is the possibility of underlying medical conditions. For instance, conditions like endometriosis or uterine fibroids can cause changes in menstrual flow and cycle regularity. If you are experiencing symptoms such as severe pain during menstruation, heavy bleeding, or unusual discharge, it would be prudent to consult a healthcare provider for further evaluation.

Regarding the specific symptoms you described, such as light bleeding at the beginning of your cycle and then a return to normal flow, this could be indicative of several things. It might be a sign of anovulation (when the ovaries do not release an egg), which can lead to irregular bleeding patterns. Alternatively, it could be related to hormonal fluctuations that affect the uterine lining, causing it to shed irregularly.

It is also worth noting that the presence of blood in your urine, as you mentioned, could be unrelated to your menstrual cycle and might warrant further investigation. Hematuria (blood in urine) can be caused by various factors, including urinary tract infections, kidney stones, or other underlying conditions. It is essential to discuss this symptom with your healthcare provider, as it may require additional testing.

In summary, irregular menstrual cycles can result from a variety of factors, including hormonal imbalances, stress, lifestyle changes, and underlying medical conditions. Given the complexity of menstrual health, it is advisable to consult with a healthcare professional who can conduct a thorough evaluation, including a physical examination and possibly laboratory tests, to determine the underlying cause of your irregular cycles and any associated symptoms. This approach will help ensure that you receive appropriate care and management tailored to your specific situation.
