Frequent urination at fixed intervals?

Hello Director Du, I am the consultant for cases 177786 and 177794.
Thank you again for your questions:
1.
Why do the RBC levels fluctuate between high and low? Will they gradually return to normal?
2.
A CT scan of the abdomen was performed on 110/9, and there were no changes compared to previous scans.
A urine cytology test was conducted in December 110, which was normal.
Therefore, is it correct for the physician not to order additional tests?
3.
Could the fixed schedule of frequent urination be related to psychological factors? Is there any connection to the COVID-19 vaccination? Does age play a role?
Thank you once again.

Hsiu, 60~69 year old female. Ask Date: 2022/01/23

Hello:
1.
Why do RBC levels fluctuate, and will they gradually return to normal? -- It is possible, but they often fluctuate again after a while.

2.
The abdominal CT on 110/9 showed no changes compared to previous scans, and the urine cytology on 110/12 was normal.
Is it correct that the physician did not order any additional tests? -- A CT scan can detect very small lesions, and if there is painless visible hematuria, some physicians may recommend a cystoscopy.
Among non-invasive tests, CT is one of the most detailed.

3.
Could frequent urination at fixed times be related to psychological factors? -- Is it possible that it is related to the COVID-19 vaccination? -- It is difficult to speculate, but could it be related to age? -- That is also a possibility.
Wishing you good health.

Frequent urination, medically known as polyuria, can be a distressing symptom that affects daily life and mental well-being. Understanding its causes, concerns, and treatment options is crucial for managing this condition effectively. Based on your detailed history, it appears that you have undergone a thorough evaluation for urinary issues, including urinalysis, urine cultures, and imaging studies. Here’s a comprehensive overview addressing your concerns:

1. Fluctuating RBC Levels
The presence of red blood cells (RBCs) in your urine can indicate various conditions, including urinary tract infections (UTIs), kidney stones, or even more serious issues like tumors. The fluctuation in RBC counts you’ve experienced could be due to several factors, including transient infections, inflammation, or even minor trauma to the urinary tract. It is not uncommon for RBC levels to vary, especially if there are underlying conditions that have not yet been fully resolved. Continuous monitoring and follow-up with your healthcare provider are essential to determine if these fluctuations will stabilize over time.

2. Diagnostic Approach
Your healthcare provider's decision to refrain from further invasive testing after normal results from CT scans and urine cytology seems reasonable, especially if there are no alarming symptoms or persistent abnormalities. The combination of normal imaging studies and cytology suggests that serious underlying conditions are less likely. However, if symptoms persist or worsen, it may be prudent to revisit this decision and consider additional evaluations, such as cystoscopy or further imaging, to rule out any overlooked issues.

3. Psychological Factors and Frequency of Urination
The psychological aspect of frequent urination cannot be understated. Anxiety and stress can significantly impact urinary habits, leading to a condition known as "nervous bladder." This is particularly relevant if you find yourself needing to urinate more frequently in specific situations, such as during social engagements or when anticipating an outing. The mind-body connection is powerful; anxiety can trigger the urge to urinate, even if the bladder is not full.
Additionally, age can play a role in urinary frequency. As individuals age, bladder capacity may decrease, and the bladder's ability to hold urine can diminish, leading to more frequent trips to the bathroom.

4. Potential Links to Vaccination
Regarding your question about a potential link between frequent urination and the COVID-19 vaccine, current evidence does not support a direct correlation between vaccination and urinary symptoms. However, some individuals may experience temporary changes in bodily functions following vaccination, primarily due to stress or anxiety related to the vaccination process itself. If you suspect that your symptoms began or worsened after vaccination, it would be worth discussing this with your healthcare provider for a more personalized assessment.

Treatment Options
Given your situation, here are some potential treatment options and strategies to consider:
- Behavioral Therapy: Cognitive-behavioral therapy (CBT) can be beneficial for managing anxiety-related urinary symptoms. This approach helps in addressing the psychological triggers that may be contributing to your frequent urination.

- Bladder Training: Gradual bladder training can help increase the time between urinations. This involves scheduling bathroom visits and gradually extending the intervals between them.

- Medication: If anxiety is a significant factor, discussing the possibility of anti-anxiety medications with your healthcare provider may be beneficial. Additionally, medications that relax the bladder may also be considered if appropriate.

- Lifestyle Modifications: Reducing caffeine and alcohol intake, staying hydrated, and practicing relaxation techniques can also help manage symptoms.

Conclusion
Frequent urination can stem from a variety of causes, including physiological, psychological, and lifestyle factors. Continuous communication with your healthcare provider is crucial for monitoring your symptoms and adjusting your treatment plan as necessary. If you find that your symptoms are significantly impacting your quality of life, do not hesitate to seek further evaluation or support from a mental health professional. Your well-being is paramount, and addressing both the physical and psychological aspects of your condition will be key to finding relief.
