Erectile dysfunction and impotence issues?
Hello, I have been experiencing this condition for six months.
Six months ago, when I wanted to engage in sexual activity, I noticed a lack of response in my genital area.
The next day, I suddenly realized that my morning erections had decreased in firmness.
I consulted a doctor who conducted testosterone and lipid tests, which came back normal.
The doctor diagnosed me with psychogenic erectile dysfunction and suggested that if there were issues, I could take erectile dysfunction medication.
I felt that the examinations were insufficient, so I sought a second opinion from another doctor.
I underwent hormone and prostate examinations, which also returned normal results, and I was again diagnosed with psychogenic erectile dysfunction.
Over the next two months, without medication, I tried to improve through exercise and mental adjustment, and I experienced some progress, but the firmness was still inadequate.
Later, I purchased daily Cialis, which worked well initially, but recently, in the past two weeks, I noticed that my morning erections have significantly decreased in firmness again.
Is this still purely a psychogenic issue?

Hello: If the issue is related to blood vessels, it is important to focus on diet and exercise to prevent vascular problems, which can lead to erectile dysfunction.
If you have the time, it is advisable to consult a urologist specializing in sexual function at a medical center for examination and testing.

More Info
Erectile dysfunction (ED) is a complex condition that can arise from a variety of causes, including psychological, hormonal, neurological, and vascular factors. Given your situation, where you have been experiencing ED for six months and have undergone tests that returned normal results for testosterone and prostate health, it seems that your condition may indeed be primarily psychogenic, as suggested by your healthcare providers.
Understanding the Causes of Erectile Dysfunction
1. Psychogenic Factors: These include anxiety, depression, stress, and relationship issues. It’s not uncommon for psychological factors to contribute significantly to ED, especially if the onset of symptoms coincided with a stressful event or emotional turmoil. The mind plays a crucial role in sexual arousal, and any disruption in mental health can lead to difficulties in achieving or maintaining an erection.
2. Physiological Factors: While your hormone levels and prostate health are normal, other physiological issues could still be at play. Conditions such as diabetes, hypertension, and cardiovascular diseases can affect blood flow and nerve function, leading to ED. Even if these conditions are not present, lifestyle factors such as smoking, excessive alcohol consumption, and lack of exercise can contribute to erectile difficulties.
3. Medication Side Effects: Some medications can also lead to ED as a side effect. If you are taking any medications, it may be worth discussing with your doctor whether they could be contributing to your symptoms.
Diagnosis of Erectile Dysfunction
The diagnosis of ED typically involves a thorough medical history, physical examination, and possibly some laboratory tests to rule out underlying conditions. Since you have already undergone hormone testing and prostate evaluations, it might be beneficial to consider additional assessments, such as:
- Nocturnal Penile Tumescence (NPT) Test: This test measures erections that occur during sleep, helping to determine if the ED is psychogenic or organic.
- Doppler Ultrasound: This imaging test can assess blood flow to the penis and identify any vascular issues.
Treatment Options
1. Psychological Counseling: Since your ED has been diagnosed as psychogenic, working with a mental health professional can be beneficial. Cognitive-behavioral therapy (CBT) can help address anxiety and stress, which may be contributing to your symptoms.
2. Lifestyle Modifications: Regular exercise, a healthy diet, and avoiding smoking and excessive alcohol can improve overall health and potentially enhance erectile function.
3. Medications: You mentioned using daily Cialis (tadalafil), which is a common treatment for ED. While it can be effective, it’s important to use it as directed and discuss any changes in effectiveness with your doctor. If you notice a decrease in its effectiveness, it may be worth exploring other options, such as different PDE5 inhibitors (like Viagra or Levitra) or other treatments.
4. Vacuum Erection Devices (VED): These devices can help create an erection by drawing blood into the penis, which can be a useful alternative if medications are not effective.
5. Penile Injections or Implants: In cases where other treatments fail, injections directly into the penis or surgical implants may be considered.
Follow-Up and Monitoring
Given the fluctuating nature of your symptoms, it’s crucial to maintain regular follow-ups with your healthcare provider. They can monitor your progress, adjust treatment plans as necessary, and provide support throughout your journey. Additionally, keeping a journal of your experiences, including any psychological triggers or changes in lifestyle, can provide valuable insights for both you and your healthcare team.
In conclusion, while your ED may currently be classified as psychogenic, it’s essential to continue exploring all potential underlying causes and treatment options. Open communication with your healthcare providers will be key in managing this condition effectively.
