Vaginal bleeding

Hello, doctor.
On January 25th, I took a bath with my partner and touched my genitals, but there was no sexual intercourse.
After returning home, I experienced continuous bleeding from my genitals, with the amount varying from heavy to light and being very unstable.
I have not had any sexual intercourse before this.
I am unsure if this is due to a rupture of the hymen or if there is another reason.
Since the obstetrics and gynecology clinics are closed for the New Year, I am seeking your thoughts here.

cc, 10~19 year old female. Ask Date: 2022/01/29

Dr. Cao Guotao reply Obstetrics and Gynecology

Hello, please observe for one more day.
If there is still bleeding, it may be necessary to undergo further examination to determine the cause.
Most hospitals will still have consultations available on New Year's Eve morning, so you can go for an evaluation.
Wishing you good health.

Abnormal vaginal bleeding can be a concerning symptom for many women, and it is important to understand the potential causes and implications. Based on your description, it seems that you experienced vaginal bleeding after engaging in intimate contact with your partner, even though there was no penetrative sexual intercourse. This situation raises several possibilities regarding the source of the bleeding.

Firstly, it is essential to consider the anatomy of the female reproductive system. The hymen, often referred to as the "virgin membrane," can be stretched or torn during various activities, including vigorous physical activity or even non-penetrative sexual contact. If the bleeding you experienced is due to the hymen being stretched or torn, it may present as light to moderate bleeding, which can vary in amount. This type of bleeding is generally not a cause for concern and should resolve on its own within a few days.

However, there are other potential causes of abnormal vaginal bleeding that should be considered. These include:
1. Hormonal Imbalances: Fluctuations in hormone levels can lead to irregular bleeding. This can occur due to stress, changes in weight, or other lifestyle factors. If your menstrual cycle has been irregular or if you have experienced other symptoms of hormonal imbalance, this could be a contributing factor.

2. Infections: Vaginal infections, such as bacterial vaginosis or sexually transmitted infections (STIs), can cause abnormal bleeding. These infections may also present with other symptoms, such as unusual discharge or odor.

3. Cervical or Vaginal Trauma: Even without penetrative intercourse, vigorous touching or manipulation of the vaginal area can cause minor trauma to the vaginal walls or cervix, leading to bleeding. This is especially true if there was any friction or if the area was sensitive.

4. Polyps or Fibroids: These are benign growths in the uterus or cervix that can cause irregular bleeding. While less common in younger women, they can still occur and should be evaluated if bleeding persists.

5. Pregnancy-Related Issues: Although you mentioned that there was no penetrative intercourse, it is still important to consider the possibility of pregnancy, especially if there was any chance of sperm exposure. Implantation bleeding can occur around the time of your expected period and may be mistaken for a menstrual cycle.

Given that you have experienced ongoing bleeding, it is crucial to seek medical attention as soon as possible, even if it is during a holiday period. A healthcare provider can perform a thorough evaluation, including a pelvic exam and possibly an ultrasound, to determine the cause of the bleeding. They may also conduct tests to rule out infections or other underlying conditions.

In the meantime, it is advisable to monitor your symptoms closely. Take note of the amount and color of the bleeding, any associated symptoms (such as pain or unusual discharge), and how long the bleeding lasts. This information will be helpful for your healthcare provider in making an accurate diagnosis.

In conclusion, while the bleeding you are experiencing may be due to a minor issue such as hymenal trauma or hormonal fluctuations, it is essential to have a professional evaluation to rule out any serious conditions. Your health and peace of mind are paramount, so do not hesitate to seek medical care.
