Chest pain, bilateral rib pain, and tenderness upon palpation of the bones beneath the breasts?

Dear Deputy Director,
Since tomorrow is New Year's Eve, my husband won't let me visit the pulmonology department, so I'm reaching out to you for help! I hope you can provide some answers!
1.
Why do I feel chest tightness and a sensation of a heavy weight pressing in the middle of my sternum when lying flat, along with some difficulty breathing?
2.
Why has my pain progressed from my right rib to my left rib, and now I have pain across my entire chest, with discomfort in the bones beneath both breasts? I usually sleep on my side, but lying down makes me feel uncomfortable and increases my chest tightness!
I have visited a neurologist twice, a cardiologist twice, undergone an endoscopy, had an EEG, and received an X-ray at a health center.
The only finding was autonomic nervous system dysregulation.
Additionally, I have been smoking since I was 19, and my addiction is quite strong!
I kindly request your assistance in addressing these concerns.
Thank you very much! I appreciate it!

Dr. Wang Lanfeng reply Pulmonology

Hello: Based on your description, some of your symptoms are more indicative of fibromyalgia, and you have undergone a series of tests that did not reveal any other abnormalities.
It is recommended that you quit smoking and maintain a regular lifestyle.
Please observe your condition for a while longer, and if your symptoms do not improve, it is advisable to seek medical attention again.
Thank you.

Understanding chest pain can be quite complex, as it can stem from a variety of causes, ranging from benign to serious conditions. Based on your description, it appears that you are experiencing significant discomfort and distress, particularly when lying down. Let's break down your concerns and explore potential causes and recommendations.

1. Chest Tightness and Breathing Difficulty When Lying Down: The sensation of having a "heavy stone" on your chest, especially when lying flat, could be indicative of several issues. One common cause is gastroesophageal reflux disease (GERD), where stomach acid flows back into the esophagus, leading to discomfort that can be exacerbated by lying down. This can also cause symptoms like heartburn and a sensation of tightness in the chest.

Another possibility is anxiety or panic attacks, which can manifest as chest tightness and difficulty breathing. Given your history of smoking, it's also essential to consider the impact of smoking on your respiratory and cardiovascular health. Smoking can lead to chronic obstructive pulmonary disease (COPD) or other lung issues, which can cause similar symptoms.

2. Pain Transitioning from Right to Left Rib Cage: The pain that has migrated from your right rib cage to your left and now encompasses your entire chest could be related to musculoskeletal issues. Conditions like costochondritis, which is inflammation of the cartilage connecting the ribs to the sternum, can cause localized pain that may feel like it spreads. Additionally, poor posture, especially when sleeping on your side, can lead to muscle strain and discomfort in the chest area.

However, it is crucial to rule out any cardiac issues, especially since you mentioned that you have seen cardiologists. If they have conducted tests such as an electrocardiogram (ECG), echocardiogram, and stress tests without finding any abnormalities, it is reassuring but does not completely eliminate the need for further evaluation if symptoms persist.

3. Smoking and Its Effects: Your smoking history is significant. Smoking can lead to chronic bronchitis, emphysema, and even lung cancer, all of which can cause chest pain and discomfort. If you haven't already, consider seeking support for smoking cessation, as quitting can significantly improve your overall health and reduce symptoms.

4. Recommendations: Given your symptoms and history, here are some steps you can take:
- Consult a Specialist: Since you have already seen neurologists and cardiologists, it may be beneficial to consult a pulmonologist (lung specialist) to evaluate any potential respiratory issues.

- Lifestyle Modifications: If GERD is suspected, consider dietary changes, avoiding large meals before bedtime, and elevating the head of your bed. Additionally, if anxiety is a factor, exploring relaxation techniques, therapy, or medication may be beneficial.

- Smoking Cessation: Engage in a smoking cessation program. There are various resources available, including counseling and medications that can help you quit.

- Follow-Up Care: Keep a symptom diary to track when your symptoms occur, their intensity, and any potential triggers. This information can be valuable for your healthcare provider in diagnosing the issue.

In conclusion, while your symptoms can be attributed to several potential causes, it is essential to continue seeking medical advice and not to ignore persistent or worsening symptoms. Your health is paramount, and addressing these concerns with a healthcare professional will help you find relief and improve your quality of life.
