Glans issues
Hello Doctor: I usually have a habit of masturbation, up to twice a day.
However, I recently noticed that once I start to get an erection and reach the point where I can no longer achieve full engorgement, I experience significant pain in the glans.
When there is no engorgement, I have no issues at all.
Could you please advise what possible causes might be behind this? (Thank you, Doctor)

Dr. Liu Mingyue reply Urology
Hello: There could be many possible causes for what you mentioned, with the most common being excessive DIY activities or issues related to the foreskin.
It is advisable to reduce the frequency of DIY activities.
If you continue to experience pain during erections after a few days, you should consult a urologist at a hospital.
Wishing you good health.

Understanding pain in the glans (the head of the penis) can be a complex issue, often requiring a thorough evaluation to determine the underlying causes. Based on your description, it seems that the pain occurs specifically during the process of erection and subsides when the penis is flaccid. Here are some potential causes and considerations regarding this condition:
1. Physiological Factors: The glans is highly sensitive, and during an erection, increased blood flow can lead to heightened sensitivity and discomfort. If there is any underlying inflammation or irritation, this can exacerbate the pain.
2. Peyronie's Disease: This condition involves the development of fibrous scar tissue inside the penis, which can cause painful erections. While it typically presents with curvature, pain during erection can also be a symptom.
3. Infections: Conditions such as balanitis (inflammation of the glans) or sexually transmitted infections (STIs) can cause pain during erections. Symptoms may include redness, swelling, or discharge.
4. Skin Conditions: Dermatitis or other skin conditions affecting the glans can lead to pain, especially during erections when the skin is stretched. Conditions like lichen sclerosus or psoriasis may also be involved.
5. Nerve Sensitivity: Increased sensitivity of the nerves in the glans can lead to pain during erections. This can be due to various factors, including previous trauma, excessive friction, or even psychological factors related to anxiety or stress.
6. Vascular Issues: Sometimes, issues with blood flow can lead to pain during erections. This could be due to vascular conditions that affect the blood vessels supplying the penis.
7. Mechanical Factors: If there is any form of mechanical irritation, such as from tight clothing or excessive friction during masturbation, this can lead to pain during erections.
Solutions and Recommendations:
- Consult a Urologist: Given the specificity and nature of your symptoms, it is advisable to consult a urologist. They can perform a physical examination and possibly recommend tests such as ultrasound or blood tests to rule out infections or vascular issues.
- Avoid Irritants: Pay attention to any products you use, such as soaps or lubricants, that may irritate the sensitive skin of the glans. Opt for hypoallergenic products.
- Modify Sexual Practices: If you notice that certain activities exacerbate the pain, consider modifying your practices. This could include reducing the frequency of masturbation or using lubrication to minimize friction.
- Pain Management: Over-the-counter pain relief medications, such as acetaminophen or ibuprofen, may help alleviate discomfort. However, consult with a healthcare provider before starting any medication.
- Monitor Symptoms: Keep track of any additional symptoms that may arise, such as changes in urination, discharge, or persistent pain. This information can be valuable for your healthcare provider.
- Psychological Factors: If anxiety or stress is a contributing factor, consider speaking with a mental health professional. Techniques such as mindfulness or cognitive-behavioral therapy (CBT) can be beneficial.
In conclusion, while pain in the glans during erection can be distressing, it is essential to approach the issue systematically. Identifying the underlying cause is crucial for effective treatment and management. Seeking professional medical advice is the best course of action to ensure proper diagnosis and care.
