Frequent urination and incomplete bladder emptying?
Hello, doctor.
I would like to inquire about an elder who is 50 years old.
He has been experiencing frequent urination for over two years, averaging a trip to the bathroom every 20-30 minutes, and he also wakes up 2-3 times at night.
He often feels that he has not completely emptied his bladder, and urination is not very smooth.
He has a habit of masturbation, doing it 2-3 times a week, and sometimes he has sexual intercourse with his wife.
Could frequent masturbation cause prostate issues or contribute to his frequent urination? He is currently seeing a urologist and taking medication, but he feels shy about discussing his masturbation habits with the doctor and whether it could affect his prostate and urination issues.
His examinations indicated that his prostate is enlarged (beyond normal values), and both a digital rectal exam and ultrasound showed residual urine in the bladder.
However, he is only on medication for treatment.
I would like to ask if the above-mentioned habits could have an impact.
Thank you for your response.

Dr. Li Xinyi reply Urology
There should be no issues with sexual activity or DIY aspects.
The prostate size being slightly larger than normal is not a concern, but the symptoms sound more severe.
This may require further examination and a discussion with the physician about the current effectiveness of the medication.
It would be appropriate to consider whether any adjustments to the medication are necessary.

Frequent urination, especially in men over the age of 50, can often be attributed to various factors, with prostate health being a significant concern. In your case, the individual in question has been experiencing frequent urination for over two years, with an average of 20-30 minutes between bathroom visits and waking up 2-3 times at night to urinate. This situation can be quite distressing and can significantly affect one's quality of life, particularly sleep quality.
The symptoms described, such as the feeling of incomplete bladder emptying and difficulty with urine flow, are commonly associated with benign prostatic hyperplasia (BPH), which is an enlargement of the prostate gland. As men age, the prostate can grow larger, leading to pressure on the urethra and causing urinary symptoms like increased frequency, urgency, and nocturia (waking at night to urinate). The fact that the individual has been diagnosed with an enlarged prostate and has undergone both a digital rectal exam and ultrasound supports this possibility.
Regarding the concern about masturbation, it is important to clarify that regular masturbation is generally considered a normal and healthy sexual activity. While some men worry that frequent masturbation might lead to prostate issues or exacerbate urinary symptoms, current medical research does not support a direct link between masturbation and the development of prostate problems or increased urinary frequency. In fact, some studies suggest that regular ejaculation may have protective effects against prostate cancer. However, if the individual experiences discomfort or pain during or after masturbation, it may be worth discussing with a healthcare provider.
The presence of residual urine in the bladder, as noted in the ultrasound findings, can also contribute to the sensation of urgency and frequency. When the bladder does not empty completely, it can lead to a cycle of frequent urination as the body attempts to expel the remaining urine. This condition can be caused by BPH, bladder dysfunction, or other underlying issues. Medications prescribed by a urologist are often aimed at alleviating these symptoms by relaxing the bladder neck and reducing prostate size, thereby improving urine flow.
In terms of management, it is crucial for the individual to continue following up with their urologist. The doctor may recommend lifestyle changes, such as reducing caffeine and alcohol intake, which can irritate the bladder and exacerbate urinary symptoms. Additionally, pelvic floor exercises may help improve bladder control and reduce urgency.
If the current medication regimen is not providing sufficient relief, it may be beneficial for the individual to discuss alternative treatment options with their urologist. These could include different classes of medications, minimally invasive procedures, or even surgical options if the symptoms are severe and unresponsive to conservative management.
In conclusion, while frequent urination and prostate health are closely linked, regular masturbation is unlikely to be a contributing factor to the urinary symptoms described. It is essential for the individual to maintain open communication with their healthcare provider about all aspects of their health, including sexual activity, to ensure comprehensive care. Regular follow-ups and adherence to prescribed treatments will be key in managing these urinary symptoms effectively.
