Post-stoma reversal

Hello: I had my stoma reversed in early December 2021, and I occasionally experience mild cramping around my navel.
Is this normal? After the reversal, I have a scar about 8 centimeters deep on my abdomen.
Will this scar be permanent? Thank you.

Xiansheng, 60~69 year old female. Ask Date: 2022/02/06

Dr. Xu Kaixi reply Surgery

Scarring after abdominal wound healing can sometimes lead to nearby neuropathic pain, which is considered a normal phenomenon.
Initially, the scar from an ostomy reversal may appear uneven, which is also normal and should improve over time.

Hello! Thank you for your question regarding post-surgical concerns, particularly related to pain and scarring after your ostomy reversal surgery in December 2021.
Experiencing occasional pain or discomfort in the area of your surgical scar, such as a slight pulling or aching sensation around the navel, can be quite common after such procedures. This pain may be attributed to several factors, including nerve regeneration, scar tissue formation, or even changes in the surrounding tissues as they heal. It’s important to note that the healing process can take time, and sensations like these can persist for months or even years after surgery.

Regarding your concern about the scar itself, it is natural for surgical scars to appear raised, discolored, or deeper than the surrounding skin initially. Over time, many scars will fade and flatten, but the extent of this depends on various factors, including your skin type, the nature of the surgery, and how well you care for the scar. While some scars may never completely disappear, there are several options available to help improve their appearance.
1. Scar Management Techniques: There are various treatments available that can help minimize the appearance of scars. These include silicone gel sheets, silicone ointments, and pressure dressings. These methods can help flatten and soften the scar over time.
2. Topical Treatments: Over-the-counter creams and ointments containing ingredients like vitamin E, aloe vera, or onion extract may help improve the appearance of scars, although scientific evidence supporting their effectiveness varies.

3. Physical Therapy: Sometimes, physical therapy can help with scar tissue mobilization, which may alleviate discomfort and improve the range of motion in the area surrounding the scar.

4. Laser Therapy: For more significant scarring, dermatological procedures such as laser therapy can be effective. This treatment can help reduce redness, improve texture, and flatten raised scars.

5. Surgical Revision: In some cases, if the scar is particularly bothersome, surgical revision may be an option. This involves removing the old scar and re-closing the skin in a way that may result in a less noticeable scar.

6. Consultation with a Specialist: It may be beneficial to consult with a dermatologist or a plastic surgeon who specializes in scar management. They can provide personalized recommendations based on the specifics of your scar and overall health.

In summary, while some discomfort and scarring after surgery are common, there are various strategies to manage and improve the appearance of scars. If your pain persists or worsens, or if you have concerns about the healing process, it is essential to follow up with your healthcare provider. They can assess your situation more thoroughly and recommend appropriate interventions to help alleviate your concerns. Remember, healing is a gradual process, and with the right care, you can achieve significant improvements over time.
