Blood in the stool?
Hello Doctor: Recently, I have noticed bright red streaks of blood in my stool, and even the water in the toilet has turned red.
However, there is no blood on the toilet paper when I wipe.
I can see a red protrusion around the anal area, which is not usually present, and I experience a slight burning sensation.
I first noticed the blood issue around early last year, but I have been drinking more than 1500 ml of water daily, and I have not experienced any weight loss or abdominal pain.
There is no relevant family history of such conditions.
I would like to ask if this bleeding could be caused by hemorrhoids, or if I need to go to the hospital for further examination.
Thank you.

When it comes to the presence of bright red blood in the stool, it is essential to consider various potential causes, with hemorrhoids being one of the most common. Hemorrhoids, which are swollen veins in the lower rectum and anus, can lead to bleeding, especially during bowel movements. The blood associated with hemorrhoids is typically bright red and may appear on the toilet paper or in the toilet bowl, as you've described. The presence of a red protrusion near the anus and a mild burning sensation can further suggest the presence of hemorrhoids.
However, while hemorrhoids are a frequent cause of rectal bleeding, it is crucial not to overlook other potential causes, especially if this is a recurring issue. Other conditions that can lead to blood in the stool include anal fissures, diverticular disease, colorectal polyps, and even more serious conditions such as colorectal cancer. Given that you have experienced this symptom for an extended period, it would be prudent to seek further evaluation from a healthcare professional.
Here are some key points to consider:
1. Symptoms and History: While you have noted that you drink plenty of water and have not experienced weight loss or abdominal pain, the presence of blood in the stool should not be ignored. Even in the absence of other symptoms, it is essential to investigate the cause of the bleeding.
2. Family History: Although you mentioned that there is no family history of related diseases, it is still important to be vigilant. Some conditions can occur sporadically and may not have a hereditary component.
3. Physical Examination: A healthcare provider will likely perform a physical examination, which may include a digital rectal exam to assess for hemorrhoids or other abnormalities.
4. Further Testing: Depending on the findings from the initial examination, your doctor may recommend further tests, such as a colonoscopy, to visualize the colon and rectum directly. This procedure can help identify any polyps, tumors, or other issues that may be causing the bleeding.
5. Management and Treatment: If hemorrhoids are confirmed as the cause, treatment options may include lifestyle changes (increased fiber intake, hydration), topical treatments, and in some cases, procedures to remove or reduce the hemorrhoids. If another condition is identified, the treatment will be tailored accordingly.
In summary, while hemorrhoids are a likely cause of the symptoms you are experiencing, it is essential to consult with a healthcare professional for a thorough evaluation. They can provide a definitive diagnosis and appropriate management plan. Ignoring the symptoms could lead to complications, so it is best to address this issue sooner rather than later.
