Drug interactions of emergency contraceptive pills?

Hello! I have been prescribed AMOXICILLIN CAPSULES 500MG and NOFLAGMA TABLETS 90MG (LYSOZYME) after having my wisdom teeth removed.
I am currently taking birth control pills (Alesse).
I am wondering if there are any interactions between these medications and whether I need to wait 7 days after stopping the antibiotics for the birth control pills to be effective again?

Niming, 20~29 year old female. Ask Date: 2022/02/12

Dr. Chen Zhiyu reply Obstetrics and Gynecology

Regarding your concerns, here is a summary of the response: The contraceptive pill you are currently taking can be continued without worrying about the drug interactions you are concerned about.
Abruptly discontinuing the medication may lead to abnormal bleeding.
Please take note of this.
Thank you for your question, and I wish you health and happiness.

When it comes to understanding drug interactions, especially in the context of emergency contraceptives and other medications, it is crucial to consider the pharmacological properties of each drug involved. In your case, you are taking Amoxicillin and NoFlagma (which contains lysozyme) while also using an oral contraceptive, specifically the brand "Aisilian."
Amoxicillin is a penicillin-type antibiotic that is commonly used to treat bacterial infections. It works by inhibiting the synthesis of bacterial cell walls, leading to the death of the bacteria. NoFlagma, on the other hand, is an enzyme that can help in breaking down bacterial cell walls and is often used as an adjunct in treating infections.

Oral contraceptives, like Aisilian, contain hormones that prevent ovulation and thus reduce the risk of pregnancy. The concern with antibiotics and oral contraceptives arises from the potential for antibiotics to affect the metabolism of contraceptive hormones, thereby reducing their effectiveness.

Drug Interaction Concerns
1. Amoxicillin and Oral Contraceptives:
- Historically, there has been concern that antibiotics could reduce the effectiveness of hormonal contraceptives. However, most studies have shown that the majority of antibiotics, including Amoxicillin, do not significantly affect the efficacy of oral contraceptives. The primary exception is rifampin and rifabutin, which are used to treat tuberculosis and can induce liver enzymes that metabolize contraceptive hormones, potentially leading to decreased effectiveness.

- Therefore, while it is always prudent to be cautious, taking Amoxicillin while on an oral contraceptive like Aisilian is generally considered safe and should not necessitate additional contraceptive measures.

2. NoFlagma and Oral Contraceptives:
- There is limited evidence suggesting that lysozyme (the active ingredient in NoFlagma) interacts with hormonal contraceptives. Given that lysozyme is an enzyme that primarily works locally in the body and does not significantly affect liver metabolism, it is unlikely to interfere with the effectiveness of your contraceptive.

Recommendations
- No Need to Wait: You do not need to wait for a period of seven days after stopping Amoxicillin for your oral contraceptive to regain its effectiveness. Since Amoxicillin is not known to significantly impact the efficacy of hormonal contraceptives, you can continue taking your contraceptive as prescribed without concern.

- Consult Your Healthcare Provider: If you have any lingering concerns or if you experience any unusual symptoms while taking these medications, it is always best to consult your healthcare provider. They can provide personalized advice based on your medical history and current medications.

Conclusion
In summary, taking Amoxicillin and NoFlagma while on the oral contraceptive Aisilian should not pose a significant risk of drug interaction that would compromise the effectiveness of your contraceptive. It is always wise to remain informed about your medications and their potential interactions, but in this case, you can feel reassured about continuing your contraceptive regimen without additional precautions. Always keep an open line of communication with your healthcare provider regarding any medications you are taking to ensure your health and safety.
